Dear Community Member:

Each year in the month of April, the City of Springfield joins the nation in celebrating public health. As part of that celebration the Public Health Luminary Award is presented to a member of the Springfield community who has become a leader in raising community awareness of a public health issue, practice and intervention. The recipient of the Luminary Award must be one who has taken public health out of its traditional setting and into the community, thereby increasing awareness and advocacy, through innovative and creative programming. Through excellence and professionalism the recipient must be one who inspires the entire community and advocate for change in the city of Springfield.

The Public Health Month Committee is now accepting nominations for such outstanding leaders.

Please take a moment to consider individuals you think are deserving of this distinction.

Complete and submit the attached nomination form by March 6, 2014.

All nominations must include the contributions made by the individual to the Springfield community and a brief biography of the nominee, or they will be null and void. This effort should not be viewed as a popularity contest.

LUMINARY AWARD NOMINATION

Please share with us who you feel deserves the Distinguished Public Health Luminary Award to be presented at this year’s Annual Public Health Month Celebration Kick-Off on Monday, April 7, 2014 at 10:00 am in Room 220, City Hall.

The person nominated should have demonstrated outstanding accomplishment in the health and human services arena. A community activist/ advocate who has “pushed” the public health agenda, or a Springfield worker/ resident who has raised our awareness about the health status indicators and who has worked to have public health care issues placed on the city’s work plan.
