Position Description
Position Title / Support Worker - Aged Care and Community Services
Position Function / Provide personal, physical and emotional support to Aged and Disability Clients who require assistance with daily tasks and assistance with culturally appropriate activities within the Aged Care Facility and the wider Community.

Key Responsibilities /
  • Provide personal care to residential and community clients as described in their care plans
  • Provide reliable, timely and flexible services that are person centred and best meet the needs of the individual
  • Problem solve issues with the individuals and the Support Worker team
  • Undertake domestic duties as required, including cleaning of resident’s rooms, bathrooms and washinglinen etc.
  • Provide support to Community clients with personal care and domestic assistance
  • Assist with meal preparation and delivery of meals into the community
  • Provide support to Residents and Community Clients with Medications
  • Assist residents and clients to attend Medical Appointments
  • Assistresidents and clients to access appropriate allied health services when required.
  • Assist residents and clients to access other community services ie: shopping, centrelinketc
  • Assist Residents and Community Clients with interpretation and advocacy
  • Ensure any documentation/reporting is completedobjectively
  • Report and address any changes to Residents assessed care needs
  • Encourage and support involvement of the family / carers in the care and support of clients
  • Work with other team members to identify and address the needs and choices of Residents and Community Clients
  • Participate in developing meaningful and culturally appropriate Activities for Residents and Community Clients.
  • Assist Residents and Community clients to participate in traditional activities hunting, fishing, bush tucker and collection of plants and flowers to dye for weaving
  • Assist clients to attend community events
  • Encourage Community Engagement with Aged Care Services
  • Access information on best practice in Aged Care service delivery
  • Support and promote skill development within the Team
  • Promote and encourage Continuous Quality Improvement
  • Ensure the requirements WH&S legislation are met and risks managed efficiently
  • Attend and participate inTeam Meetings
  • Participate in annual performance review with Manager, Aged Care
  • Perform other duties as directed by the Manager, Aged Care
Information Systems
  • Adhere to the Information technology policies of Malabam Health Board
Professional Development
  • Participate in appropriate professional development to maintain and develop professional expertise, skill and knowledge
Mandatory Requirements
  • Satisfy criminal history check
  • Have clearance as required by section 187 of the Care and Protection Act to be engaged in child-related employment; possess NT Ochre Card
  • Confidentiality is a mandatory requirement; any breach of this requirement may compromise ongoing employment
  • Compliance with all policies and procedures of Malabam Health Board and Mala’la Flexible Aged Care and Community Services
  • Report to the Chief Executive Officer immediately any personal criminal charges or convictions that arise
  • Participate in annual fire drill
  • Participate in annual performance review with the Manager – Malal’a Aged Care
Work, Health and Safety
  • Be familiar with WH&S policies and procedures; apply guidelines and procedures
  • Report safety hazards within the workplace
  • Identify and assist in rectifying safety hazards within the workplace environment

Essential / Selection Criteria /
  • Certificate 3 – Individualised Support or willingness to obtain Certificate 3 within 12 months of commencing duty
  • Experience working in Aged Care and or Disability
  • Preparedness to work a variety of shifts including evenings, weekends and public holidays as required
  • Good communication skills
  • Ability to work independently with minimum supervision
  • Current manual driver’s licence
  • Current First Aid
  • Satisfactory criminal history check
  • Current Ochre card holder

Desirable / Selection Criteria /
  • Experience working within an Aboriginal Medical Service
  • Experience living in a remote Aboriginal community
  • Demonstrated ability to engage and work effectively within a remote Aboriginal community
