II.Reason for proposed action

(Include a detailed justification/rationale for the proposed change describing, with attention to accreditation and licensure requirements,enrollment changes, educational benefits, and employment demand, as appropriate, which will result.

To satisfy IBHE requirements, enrollment projections for the next five years as well as job outlook statistics must be provided for the programmatic change.Evidence of the enrollment projections should be included. Also describe any other programmatic changes that will result from the proposed RME.

III.Program delivery mode

(Indicate whether the proposed change defines the program as distance education or off campus. Please check with SIU Extended Campus for RME requirements: . Currently established programs moving to an online format may require different forms)

IV.Anticipated budgetary effects

(Staffing neutrality required. Please include non-staffing costs to be absorbed by the unit and others, including Morris Library, Enrollment Management, etc.. ‘No effect’ is not feasible. At the very least, mention ‘minimal costs’ such as editing websites, changing recruitment material, catalog copy, etc.)

V.Arrangements to be made for (a) affected faculty, staff and students; and (b) affected equipment and physical facilities

(Eliminating programs with student enrollment requires a teach–out plan. Plans for teaching out curriculum, advisement of students, and timeline for the teach out should be included)

VI.Will other educational units, curricula, or degrees be affected by this action?

(If this proposal requires course work from another department, creates coursework similar to current courses in other departments, or is an interdisciplinary offering, solicit and attach concurrences for each affected program. An email or memo of concurrence from the Department(s) or Dean(s) of the College will suffice)

VII.Assessment of Student Learning Outcomes

(Attach the current major program’s updated assessment plan and latest report for the program being modified. Example: If you are proposing to add a ‘specialization’ to History, attach the current ‘History’ assessment plan. Current plans can requested via email to or by calling 453-7653.

VIII.Catalog copy to be deleted or added

(If proposal approval will result in any changes to the undergraduate or graduate catalog, a form 90A must be included. Please provide a clear copy of catalog language to be added or modified)

IX.The requested effective date of implementation

(After final approval, notification of the effective date will be provided to the Dean and Department. Typically, the form 90A will be processed immediately after approval for the next catalog cycle. The change must receive final approval by October 1 to be included in the next academic year catalog that begins the following Summer semester. Exception must be approved by the Provost)
