PROM DANCE GUEST REQUEST
The completed form must be turned in to the Principal’s Secretary no later than Wednesday, April 11th by 2:30 pm.
To be considered, the application must have a clear copy of a valid driver license or photo id of the guest attached and be complete.
All guest applications are subject to approval or disapproval by the principal or designee. No middle school students or anyone over the age of 20 will be considered. Providing false information on this application will result in the request being denied.

As a guest of a student, I understand the following:
ü All guests must comply with all school and Code of Conduct requirements.
Dress Code for Young Men: Dress pants, dress shirt and dress shoes. A tie and jacket is strongly recommended. All pants must be securely worn around the waist. No “polo” style shirts will be allowed. Athletics shoes/sneakers (i.e. Nikes, Adidas, etc.), skate shoes (i.e. Vans, Airwalks, etc.) or workboots will not be permitted. The principal's/designee's decision is FINAL.
Dress Code for Young Women: Must wear a dress, pant suit or gown that is in good taste. The outfit may not be made of see-through material unless the skirt/top underneath the see-through material is within the parameters of dress code (no more than 4-inches above knee, neckline that forms between left & right armpit). Strapless dresses are allowed; however, no open styles, which show the entire stomach area are permitted. All dresses must cover the midriff area completely. The sides of the midriff area must also be covered. Front necklines shall not be lower than a line formed between the right and left armpit. This guideline governs cleavage and the principal's/designee's decision is FINAL. The dress length and all slits cannot exceed more than four inches above the knee.
Dress Code for Both Young Men and Young Women: Students dressed inappropriately will be turned away at the door. Students will not be admitted wearing cargo pants, sneakers, jeans/denim, khakis, Dickies (or other casual pants), t-shirts, torn clothing, chains/spikes, sexually implicit or explicit clothes, shorts, hats and head coverings, bandanas, or items referencing tobacco or alcohol products. The principal's/designee's decision is FINAL.
ü Students must be in good standing at their school.
ü Guests must present a driver license or school ID for identification for admission.
ü This dance is a tobacco free activity. The use of tobacco products is NOT permitted.
ü Students are not allowed to re-enter the dance upon leaving.
ü Students must vacate the premises within 30 minutes of the dance ending.
ü All students and guests must enter the prom by 9pm. The doors to the Prom will be closed at 9pm, and no entry into prom will be allowed after 9pm.
ü Guests will be removed from the premises with no refund for improper conduct, violations of rules, and/or refusal to comply with requests from staff and chaperones.

Students may bring only one guest. A ticket for a guest may not be purchased until the request has been approved. No requests will be accepted at the door. Both the WCHS student and guest will be removed from the premises with no refund if it is determined that a guest is in violation of any school expectation.
