VOLUNTEER WAIVER FORM

I, ______, acknowledge and understand that as an unpaid volunteer for the Project Self-Sufficiency Volunteer Car Repair Program, I will be asked to perform repair and maintenance work on the vehicles of Project Participants at the City of Fort Collins Fleet Services Repair Shop located at 906 Vine Street in Fort Collins. I hereby represent that I am a qualified mechanic capable of properly performing maintenance and repairs to passenger vehicles, and agree that I will not undertake maintenance and repair work beyond the scope of my expertise and capabilities.

I EXPRESSLY UNDERSTAND AND AGREE THAT NEITHER PROJECT SELF-SUFFICIENCY OF LOVELAND-FORT COLLINS, NOR ANY OF ITS OFFICERS, AGENTS, VOLUNTEERS OR EMPLOYEES, NOR THE CITY OF FORT COLLINS, COLORADO, A MUNICIPAL CORPORATION, NOR ANY OF ITS OFFICERS, AGENTS, VOLUNTEERS OR EMPLOYEES, SHALL BE HELD RESPONSIBLE FOR OR MADE THE SUBJECT OF ANY CLAIM SEEKING TO ASSESS DAMAGES OR LIABILITY FOR OR ARISING FROM PERSONAL INJURY OR PROPERTY DAMAGE OR LOSS OF ANY OTHER SORT TO MYSELF OR ARISING FROM THE VOLUNTEER CAR REPAIR PROGRAM, FROM MY PRESENCE OR PARTICIPATION IN THE VOLUNTEER CAR REPAIR PROGRAM, OR FROM ANY ALLEGED DANGEROUS CONDITION PRESENT AT OR IN THE VOLUNTEER CAR REPAIR PROGRAM FACILITIES AT 906 VINE STREET, FORT COLLINS.

I further understand that Project Self-sufficiency of Loveland-Fort Collins is an exempt organization under Sections of 501(a) and 501(c)(3) of the Internal Revenue code, and that as an unpaid volunteer performing services for Project Self-sufficiency of Loveland-Fort Collins. I have been granted immunity from liability for my volunteer services under the Colorado Volunteer Service Act (§ 13-21-115.5,C.R.S.)

I certify that I am legally an adult, 18 years of age or older, and therefore able to participate in the volunteer Car Repair Program. I acknowledge that no payment will be given to project Self-Sufficiency, any volunteer, or the City of Fort Collins for the services rendered. Any money paid will be for the purchase of repair parts and fluids, and payment in any form for the services provided shall not be permitted.
