1  Purpose

The purpose of the Issue Log is to summarise all of the Project Issues, their analysis and status.

The Issue log is best maintained in tabular format as below. This can be maintained in an MS Word document or an MS Excel spreadsheet

Issue Number / Type
(C / P / G) / Author / Date Identified / Description / Date of Last Update / Status/Update Comment
Legend
Issue Number: / A serial number given to each issue so as to avoid the need for long-hand reference to each issue
Issue Type / Typical issue types are; Change Request (C), Problem (P) or General (G)
Author / The person that bought the issue to the attention of the project team
Date Identified / The date that the issue was entered into the Issue Log
Description / The original description of the Issue
Date of Last Update: / As the project moves forward the issues will be addressed and a running log is kept of the changes in the status of the issue. This is vital for audit purposes
Status Update: / The latest commentary on the issue status. As each comment is recorded the date of last update is added so as to create a full audit trail of the activity on the issue.
