Through Vitality, they help you understand your health, and they suggest ways you can improve it. Members can get healthy with 50% off monthly gym membership fees at Virgin Active¹,gain bonus Nectar points if they buy certain healthy foods from Sainsbury’s and sign up to Weight Watchers Monthly Pass for up to 12 months for just £30.

They help you track your progress by giving you points for doing healthy things. And, to keep you motivated, they give you incentives for making positive changes to your health, including 10% to 50% cashback when you shop in-store and online at LloydsPharmacy², a free cinema ticketevery week at Vue or Cineworld3 and up to 25% cashback on the lowest available price for luxury hotels with Mr & Mrs Smith4.

They are the only health insurer to offer non-smoker’s cash back

PruHealth believe you should benefit from living a healthy lifestyle. They are the only health insurer to reward non-smokers with cashback payments. You could get as much as 15% of your annual premium back in cash. All youneed to do is make sure every adult on your plan takes a Vitality Healthcheck, which willconfirm that you have all been tobacco and nicotine free for at least 12 months.

PruHealth look after you if you get ill

Through their health insurance, PruHealth make sure you get the best possible medicalcare if you get ill. They give you access to the latest drugs and treatments, that are covered on your plan. Whenthey say ‘Full Cover’ they mean it. And you won’t just get looked after when you’re inhospital. Care starts the minute you call them. You’ll speak to someone who listens, whounderstands and who’ll do everything they can to help.

How your plan works

This includes PruHealth’s Vitality programme, In-patient and Day-patient Cover, Full Cancer Cover and other added benefits including Assistance at Home, Childbirth Cash Benefit and Private Ambulance.

As discussed, the underwriting option you choose is important as it could affect the level of cover you receive from PruHealth. You have selected the following option as it best suits your current circumstances:

  • Full Medical Underwriting – You are happy to tell PruHealth about your medical history. They might need to exclude some pre-existing conditions from your cover which can include related conditions. But any personal medical exclusions that are applied will be detailed on your certificate of insurance.

OR

  • Moratorium Underwriting – PruHealth don’t look at your medical history when you apply, they’ll tell you whether you’re covered at the time you make a claim. You’ll be covered for any eligible medical conditions you’ve never had before as well as eligible medical conditions that you had more than five years before you took out the plan. But if you’ve had any conditions in the five years before the plan cover start date, PruHealth usually won’t cover them. They can only cover eligible medical conditions like these if, after your plan has started, you go for two continuous years without having any treatment, medication or advice for these pre-existing conditions or any directlyrelated conditions.

OR

  • Continued Personal Medical Exclusions (Switch) Underwriting – As you want to keep the underwriting terms you have with your existing provider, PruHealth will ask you some questions about your health and any recent claims you’ve made. They will also ask to see a copy of your current insurance certificate. In some cases they may need to apply further personal medical exclusions.

No-claims discount

You could be eligible for a no-claims discount of up to 65%.For each plan year in which nobody on the plan claims, you'll move up one level on the scale. If someone on your plan makes a claim, your no-claims discount in the next year will move down two levels for each person who claims, however many claims they make.

By engaging with Vitality you could qualify for an enhanced Vitality no-claims discount of up to 80%.Engaging with Vitality also means your no-claims discount could be protected.

What no-claims discount you can start with

If you don’t already have health insurance,you’ll start on a no-claims discount of 65%.

If you already have personal health insurance from another provider and want to switch to PruHealth, they'll need to work out how much no-claims discount you can start on. Please note in some situations it may not be possible to switch to PruHealth. To do this, they'll look at how many relevant claims you've made, and how long you've been insured. You can still get up to the starting no-claims discount of 65%.

Affordability

You have indicated that you have £XX of your monthly income which could be used to address your health insurance needs. This level of commitment is affordable now and in the future.

Your new plan will start when your application has been accepted by PruHealth. Please be aware that in certain circumstances PruHealth may be unable to offer cover.

Important information

  • If you already have a private medical insurance plan but wish to move to PruHealth, please do not cancel your existing cover before your PruHealth plan is in place.
  • This recommendation has been made on the basis that:
    - You will be aged between 18 and 79 at the plan start date

< - Your spouse / partner lives at the same address as you and will be aged between 16 and 79 at the plan start date>

< - Your children, including adopted children, will be under the age of 20 (or under the age of 25 if ‘switch’ underwriting has been chosen), at the plan start date>
- All applicants live in the UK for at least 180 days in each plan year

  • The reasons for recommending PruHealth have been detailed above but you need to be aware that it would be possible to obtain similar levels of cover using other providers and products available in the UK market. The Vitality programme, however, is unique to PruHealth and while the level of cover may be replicated across other providers the benefits of the Vitality programme cannot be found elsewhere.
  • Please ensure you answer any questions asked by PruHealth in the application process honestly and in full. If you do not provide factually correct information PruHealth may not be able to pay a future claim.
