FORMS NEEDED TO APPLY FOR A VIRGINIA TEACHING LICENSE

Create a complete Initial License Packet using this list as a guide and send a complete packet verifying licensure eligibility to your Licensure Specialist within 30 days of your on-boarding orientation date.

(IF YOU HAVE BEEN HIRED IN THE FOLLOWING POSITIONS – YOU DO NOT HAVE TO COMPLETE THE LICENSE APPLICATION PROCESS: OCCUPATIONAL/PHYSICAL THERAPIST, BEHAVIORIAL SPECIALIST, SIGN LANGUAGE INTERPRETER, VOCATIONAL RESOURCE OR SPEECH PATHOLOGY)

1.  APPLICATION FOR A VIRGINIA TEACHING LICENSE (STANDARD) – COMPLETE THIS APPLICATION UNLESS YOU ARE A SPECIAL ED TEACHER THAT HAS NOT COMPLETED A SPECIAL ED PROGRAM

2.  SPECIAL EDUCATION PROVISIONAL LICENSE – ALL SPECIAL ED TEACHERS MUST COMPLETE THE SPED PROVISIONAL LICENSE APPLICATION UNLESS YOU HAVE COMPLETED AN APPROVED SPED PROGRAM

APPLICATION FEE - The application fee can be paid by certified check, cashier’s check, money order, or a personal check. There is a $25 processing fee if a personal check is returned. In-state fee: $50 and out-of-state fee: $75.

Make payments payable to: Treasurer of Virginia.

TRANSCRIPT OF CREDIT – You must request that updated OFFICIAL TRANSCRIPTS of ALL undergraduate and graduate study be sent directly to you and then sent as part of the Complete Licensure Packet to the Office of Certification. (Please ensure that transcripts confer your degree) -Opened official transcripts with a stamp or seal will be accepted. Internet, eSCRIP-SAFE or third party transcripts will not be accepted. No highlighting allowed.(Official transcripts submitted for the employment application process are not maintained by the Office of Certification, you must include official transcripts with your licensure packet.)

COLLEGE VERIFICATION FORM – Please note that this form is required only if you completed an approved preparation program in education, if this applies - the form must be completed by the designated official (usually the certification office of the college of education) of the college from which you received your most recent degree in education. Part I should be completed by you and Part II should be completed by the college official and returned directly to you. (You may submit a copy of a full, valid out-of-state license with no deficiencies for the completed program area in lieu of the College Verification Form if applicable.)

REPORT ON EXPERIENCE – Please note that this form is required only if you have completed the required experience to be exempt from the professional teacher’s assessment test (PRAXIS II) , Virginia Communications and Literacy Assessment (VCLA) and Virginia Reading Assessment (VRA) or Reading for Virginia Educators (RVE) - Request the completion of this form by the appropriate accredited non-public or public school division official if you have completed at least three years of full-time teaching experience in kindergarten and/or grades 1-12 in a state outside of Virginia.

OUT–OF-STATE-TEACHING LICENSE (S) – Please submit a photocopy of your full, valid out-of-state license. (You must also complete the application process for the Virginia Teaching License and provide official transcripts and any other applicable forms.) The VDOE does not accept internet versions of an out-of-state license unless the state will no longer issue a hard copy license.

*VIRGINIA COMMUNICATIONS AND LITERACY ASSESSMENT – if met, please send a photocopy of your assessment score sheet. Please visit the VCLA website for information on content, cost and registration @ www.vcla.nesinc.com

*PRAXIS II ASSESSMENT– if met, please submit a copy of your PRAXIS II scores unless you have been exempted by completing & returning the Report on Experience Form signed by your previous employer (3 or more years teaching outside of Virginia) AND you hold a full, valid out-of-state license with no deficiencies. *READING FOR VIRGINIA EDUCATORS (RVE) –if met, please submit a copy of your assessment score sheet. This assessment is required of all candidates applying for licensure as Prek-3, Prek-6, Special Education teachers and Reading Specialists. (Early childhood, speech-language pathologists and teachers of severe disabilities are exempt from this requirement) Please visit the Educational Testing Services (ETS) website for registration information @ www.ets.org/praxis/register

*A 3 year provisional license will be issued if any of the above assessments specific to your endorsement area have not been met.

CHILD ABUSE RECOGNITION TRAINING – This online course takes about 1 hour and is available at no cost: http://www.dss.virginia.gov/family/cps/mandated_reporters/cws5691/index.html

After completion of the course, print your certificate, write your SSN on the certificate and send to the Office of Certification with your “Complete Packet.” (Required for initial licensure – no other child abuse recognition training will be accepted)


EMERGENCY FIRST AID, CPR AND AED TRAINING – include a certification of completion clearly indicating completion of all three components. Documentation must include your full name, date training was completed, signature and title of the organization or group that provided current national evidence-based training. (A hands-on assessment is not required for licensure purposes however may be required if the position you have been hired to teach requires certification such as a Health and PE Teacher or Athletic Trainer position – check with your advisor.) If you have not already completed the required training. on-line opportunities requiring a fee, to meet the licensure requirement are listed on the Certification website @ http://pwcs.certification.schoolfusion.us/ then choose Emergency First Aid, CPR and AED Training.(On-line training opportunities will take from 2.5 to 5 hours to complete, depending on which one you choose.)

INDUSTRY CERTIFICATION - IF YOU ARE SEEKING AN ENDORSEMENT IN BUSINESS AND INFORMATION TECHNOLOGY, FAMILY AND CONSUMER SCIENCES, TECHNOLOGY EDUCATION OR MARKETING EDUCATION – Refer to Certification website for a listing of issuing organizations for your endorsement area and include a copy of the certification with your Licensure Packet.

The above mentioned forms and official transcripts should be sent as a complete packet to: Prince William County Public Schools, New Teacher Certification Office, PO Box 389 Manassas, Virginia 20108 within 30 days of your employment date. Individuals eligible for a Virginia Teaching License through a reciprocity system must complete the application process.

Please refer to Regulation 513.01-1 for licensure guidelines indicating that your completed application packet must be received within 30 days from first date of employment or orientation date, whichever comes later.
