Use the document checklist below to organize the documents you need to compile to establish your eligibility for the Deferred Action for Dreamers program

Proof of My Identity/Date of Birth/Qualifying Age

o  Birth Certificate

o  Passport

o  National Identity Document/Cedula

o  Baptismal Record

Proof that I Had Five Years of Continuous Residence in the U.S. as of June 15, 2012

o  School Records

o  Financial Records

o  Employment Records

o  Tax Records

o  Medical Records

o  Church Records

o  DMV Records

o  Entry Documents (I-94, stamp in passport)

Proof That I Am In School, Have a High School Diploma or GED, or That I Was Honorably Discharged From the U.S. Coast Guard or U.S Armed Forces

o  School Transcript, Report Cards

o  High School Diploma

o  GED Certificate

o  Military Discharge Papers

Proof That I Do Not Have A Disqualifying Conviction And Do Not Pose a Danger to Public Safety/National Security

o  Juvenile Court Records if any

o  Criminal Court Records, if any (arrest records, court records)

o  *If you have never been arrested and/or charged with any offense, you do not need to submit any documents to establish this. Like all deferred action applicants, you will be required to go to a biometrics appointment and have your fingerprints taken to determine if you have any arrest record

Fees

o  Check or money order in the amount of $465 for Employment Authorization and Biometrics. Make check or money orders payable to: U.S. Department of Homeland Security

o  I will be requesting a fee exemption:

o  Proof of my income < 150% of poverty AND

o  Proof I am under 18, homeless, in foster care, or w/o parental support OR

o  Proof I cannot care for myself because of serious chronic disability OR

o  Proof I have > $25K in unreimbursed medical expenses in past year
