NotesA School COSHH form in Word is available on the School Server.

Available from the Health and Safety Unit.

Guidance on making an assessment is given in Making a Chemical Hazard and Risk Assessment.

Guidance is also available from Guidance on Completing the Chemical Hazard and Risk Assessment Form.

Use a continuation sheet to expand any section of this form in hard copy version.

1 / LOCATION OF THE WORK ACTIVITY / Lab G10
2 / PERSONS WHO MAY BE AT RISK
List names where possible / All occupants of the lab (Laura Tate, Jo Field, Dong Ling Zeng, Rita Godfrey)
3 / ACTIVITY ASSESSED / Preparation of bacterial plasmid DNA using Sigma’s Genelute kit PLN-70
4 / MATERIALS INVOLVED / Attach copies of data sheet(s)
NAME / AMOUNT
max/stored / HAZARD / RISK PHRASES / HAZDAT NO*** / BIOSCIENCESNO***
Resuspension solution
(S1) / 15.5ml / IRRITANT / R36/37/38 Irritating to eyes, respiratory system and skin
Lysis buffer (S2) / 15.5ml / IRRITANT / R36/37/38 Irritating to eyes, respiratory system and skin
Neutralisation/Binding buffer (S3) / 30ml / HARMFUL
IRRITANT / R22 Harmful if swallowed
R36/38 Irritating to eyes and skin
Optional wash solution (OWS) / 40ml / FLAMMABLE
HARMFUL
IRRITANT / R10 Flammable
R22 Harmful if swallowed
R36/38 Irritating to eyes and skin
R67 Vapours may cause drowsiness and dizziness
Wash solution concentrate (WS) / 15ml / IRRITANT / R36/37/38 Irritating to eyes, respiratory system and skin
Elution solution (ES) / 8ml / NSH
Ribonuclease A solution / 0.1ml / No risk phrases given but avoid skin contact and inhalation as a precaution
Ethanol (not supplied in kit) / 2.5 litres / HIGHLY FLAMMABLE / R11 Highly Flammable
Wash solution diluted with ethanol (80% ethanol is the final conc) / 75ml / HIGHLY FLAMMABLE
IRRITANT / R11 Highly flammable
R36/37/38 Irritating to eyes, respiratory system and skin
5 / INTENDED USE**
Give brief details and attach protocol/instructions / See attached sheet
6 / RISKS to HEALTH and SAFETY from INTENDED USE
From personal exposure or hazardous reactions. Refer to OELs, flash points, etc., as appropriate. Are pregnant women, breast-feeding mothers especially at risk?
There is a fire risk for the ethanol and the wash solution once ethanol has been added.
There is no need to use the optional wash solution for our work therefore there is no risk from using it
although there may be a risk in the event of dropping the container.
There is a risk of contact with any of the other solutions
7 / CONCLUSIONS ABOUT RISKS
Is level of risk acceptable? Can risk be prevented or reduced by change of substance/procedure? Are control measures necessary?
One risk is removed by not using the OWS but the risks from the other components need to be controlled
8 / CONTROL MEASURES
Additional to Good Chemical Practice
Keep the ethanol and the diluted wash solution away from all sources of ignition
Wear protective clothing, gloves and goggles when working with the other solutions
9 / INSTRUCTION/TRAINING
Specify course(s) and/or special arrangements.
10 / MONITORING
Performance of control measures,
Personal exposure / Health Surveillance
11 / WASTE DISPOSAL PROCEDURE
See School Server for Approved Procedure Document on specific Chemical Waste Disposal.
The precipitated cell debris from step 4 can be discarded in the bin.The flow-through liquid obtained after spinning the samples through the column (step 5) can be discarded down the sink with plenty of water. The flow-through liquid from step 7 should be disposed of in the non-halogenated waste solvent bottle.
12 / REVIEW
Enter the date or circumstances for review of assessment (maximum review interval 5 years)
June 2006
13 / EMERGENCY ACTION
TO CONTROL HAZARDS / To stabilize situation eg spread absorbant on liquid spill; eliminate sources of ignition, etc.
Turn off all sources of ignition. Contain the spill with paper towels. The spill is unlikely to be big enough to warrant using the spill kit. The towels can then be bagged and thrown in the bin.
TO PROTECT PERSONNEL / Evacuation, protection for personnel involved in clean-up, Special First Aid
Protective clothing, gloves and goggles should be worn when clearing up. The room should be ventilated if
ethanol or diluted WS has been spilled to help get rid of any vapours.
TO RENDER SITE OF EMERGENCY SAFE / Clean-up/decontamination
Wash site with plenty of water.
