In an effort to assess the Recycled Content of our product and provide any LEED APs with the appropriate documentation for LEED Materials and Resources credit 4: Recycled Content and Materials and Resources credit 5: Regional Materials. Please provide us with the following information on your company letterhead with appropriate signature. We will keep this on file for all future requests for LEED for this particular product.

Precast Concrete Recycled Content Request

Directions for use:

1.  Save to your hard drive in an appropriate location.

2.  Click on the “Sample Letter” text and delete it.

3.  Add your company logo to the letter by selecting Picture from the insert toolbar. Be sure to right click on the image and select Behind Text under the Text Wrapping selection. This will allow the logo to be placed in any location on the document.

4.  Fill in the appropriate blank information boxes.

a.  Who is the document to?

b.  Who is the document from?

c.  Make sure to supply in the Title box the name of the product you would like the appropriate data to cover.

5.  Modify the table as necessary.

To add/delete an additional row/column:

Right click inside the table and select insert. It will give you several options to include a new row or column.

6.  Fax/E-mail to all producers. Be sure to delete the Directions for Use page and resave prior to e-mailing the form.

7.  Be sure to keep these on file as the LEED AP will need a copy to upload to the GBCI Web site in order to obtain project certification.

8.  Be sure to routinely ask producers for an updated Recycled Content Request form if their recycled content has changed.
