Practicum Learning Contract

PRACTICUM LEARNING CONTRACT BETWEEN SINGAPORE POLYTECHNIC,______, AND ______AS REQUIRED FOR THE FULFILMENT AND AWARD OF THE DIPLOMA IN ADULT AND CONTINUING EDUCATION (DACE)

  1. The DACE Practicum is intended as a partial requirement for the award of the DACE. It provides the means by which the DACE learner to apply the knowledge and skills from the DACE units in authentic Continuing Education and Training (CET) work setting. The DACE trainee (referred to as Practicum Learner) would be observed and evaluated by an observer-supervisor (referred to as Practicum Supervisor) from the DACE Approved Training Organisation (referred to as the ATO), to meet specific requirements of the DACE Practicum within the authentic workplace environment provided by the Practicum Host Organisation (referred to as PHO).

Objectives of the DACE Practicum Contract

  1. This Practicum Learning Contract serves to express the commitment and obligations of the three parties, namely the ATO, the PHO and the Practicum Learner to one another, in entering this Practicum arrangement, which shall commence on the ____day of ______of the year 2012.

Roles and Responsibilities

  1. The detailed roles and responsibilities of ATO, PHO and the Practicum Learner are outlined in the document Practicum Policy Guide. All 3 parties should familiarize themselves with the structure and requirements of the practicum as detailed in the Guide.

(ATO, PHO and Practicum Learner can add-on additional roles and responsibilities in accordance to the context and operating environment requirements.)

Practicum Plan

  1. The PHO, Practicum Supervisor and the Practicum Learner shall come to mutual agreement on a Practicum Plan in accordance with the requirements of the DACE Practicum.

4.1This Practicum Plan shall provide the schedule details of the sessions in which the Practicum Learner may either:

4.1.1understudy the training delivery and conduct of assessment as practised in the PHO, or;

4.1.2be allowed to personally undertake the conduct of the training session to fulfill the Practicum requirements.

4.1.3design, develop, review and/or contextualize programme materials as assigned by PHO.

4.2The Practicum Plan shall also include any other scope of work required of the Practicum Learner as part of the Practicum learning experience, mutually agreed with the Learner and the Practicum Supervisor. A suggested template for the Practicum Plan is attached.

4.3It is the responsibility of the PHO, through the Practicum Coordinator, to provide these opportunities and coordinate the communication of these opportunities to the Learner and the Practicum Supervisor through the Practicum Plan. The PHO is committed to confirm the Practicum Plan, in consultation with the Learner and Practicum Supervisor by (date) ______.

Appeal Procedure

  1. Any disputes or grievances arising from the Practicum arrangement or review raised by any one of the parties should be directed to WDA through the ATO. WDA shall investigate the matter and convene a separate session with the relevant parties to seek an agreed outcome.

Acceptance of Practicum Learning Contract

  1. All the 3 parties of this Practicum arrangement accepts the terms of the Practicum as stipulated in this contract, through the individuals or authorised representatives entering this agreement.

Termination of Contract

  1. The contract will be held to be in force as long as the obligations and deliverables listed within the Policy Guide, of any of the parties remain outstanding except on :

7.1The receipt and acknowledgement of the formal notice of withdrawal of any one of the 3 parties from this contract (See penalty clause in paragraph 8); or

7.2Upon the passage beyond 12 months from the date of the signing of this contract.

Penalty for Premature Termination and Withdrawal

  1. In the case of disruption of this agreement, the disrupting party of this agreement commits to paying all cost to the other 2 parties that they have in reality and reasonably incurred by their participation in this agreement and as a result of the pre-mature termination of this agreement. In specificity and minimally, the disrupting party shall make good any losses suffered as a result of the withdrawal or loss of any WSQ funding that would have been payable to the other 2 parties at the successful completion of the DACE Practicum. The parties may anticipate and delineate these costs in a separate document at the point of the signing of this agreement.
