General Summary:

The Home Health Nurse provides professional skilled nursing care to residents of Rusk County, in the client’s place of residence.

  1. SKILL

Knowledge: (Level 2)

The position requires a thorough knowledge or training in a profession, or in a specialized technical field such as nursing/specialized health care. The position is equivalent to a bachelor’s degree.

  • Must maintain current State of Wisconsin Board of Nursing License.
  • Perform comprehensive assessments that encompass environmental, psychosocial, physiological and health related behaviors.
  • Develop and manage patient cares according to nursing process: Assess, Diagnose, Plan, Implement and Evaluate.
  • Must maintain current Wisconsin driver’s license and auto insurance.

Experience: (Level 2)

One year skilled nursing/nursing home/home care experience required.

  1. RESPONSIBILITY

Supervision: (Level 3)

The Home Health Nurse is considered a technical expert in their field and has staff authority as a technical expert.

  • Supervises home health aides. Assesses patient needs regarding home health aide services, assigns required tasks, supervises care provided and follows up with appropriate actions.
  • Supervises patient/family or any non- professional participating in the patients’ care regarding nursing care needs and other related problems of the patient at home.

Administrative: (Level 1)

Administrative responsibilities are uncomplicated and routine. Position involves some functional responsibility, generally limited to providing functional advice and guidance to a limited group.

  • Can assigns home health aide activities, handle patient referrals, maintain accurate charting/documentation related to service.

Customer Relations: (Level 3)

Customer/public relations involve the explanation of the complex program or organizational requirements, rules, guidelines etc…

Government Relations: (Level 1)

Minimally involved with government relations. Applies understanding of governmental rules, regulations and guidelines in performance of their job duties.

Safety of Others: (Level 2)

Requires oversight of the public served as it relates to moderate exposure to physical hazards. The position requires routine training for common work place or public safety concerns.

Complexity of Duties: (Level 3)

Duties are somewhat diversified nature involving a variety of applications where the employee is responsible for choice of action within prescribed limits or precedents.

  • Makes the initial evaluation visit to the patient within the required Medicare guideline of 48 hours or per physician’s ordered start of care date. Develops and implements the nursing care plan for the patient through planning with the patient, the family and the referring/primary physician(s). Refers to community services as needed.
  • Regularly reevaluates the patient’s needs.
  • Provides the services requiring skilled nursing care and initiates appropriate preventative and rehabilitative procedures requiring professional nursing skills in all agency programs as assigned.
  • Teaches and counsels the patient and family members, or any non-professional participating in the patient’s care regarding the nursing care needs and other related problems of the patient at home.
  • Maintains computerized records and reports as required by the agency as well as those required by Medicare and Medical Assistance.
  • Participates in education and in-service programs to update and improve professional skills and requirements.
  • Promptly informs the physician and other personnel participating in the patient’s care of changes in the patient’s condition and needs.
  • Provides nursing care outside the normal office hours as a patient condition requires or per physician’s orders.

Environmental: (Level 2)

Working conditions include moderate environmental conditions that frequently impact physical comfort. Working in this position requires specialized clothing or use of common personal protective equipment. (BSI Protocols, Infection Control Protocols, Universal Precautions)

Physical Demand: (Level 2)

The job entails moderate physical demands typically found in trade work, with some exposure to work place hazards. This position requires regular lifting, moving, bending, twisting, etc… (This position provides assistance with transfers of the client, range of motion exercises, positioning of the client and in some situations lifting the client.)
