POST INFORMATION
Post: Specialty Doctor in Breast Surgery
Base:Queen Margaret Hospital, Dunfermline
You will join a well established team of 2Consultant Surgeons, 1 Staff GradeSurgeon and 3Specialist Nurses who provide elective and emergency services in Fife.
This is a full-time post based at Queen Margaret hospital, Dunfermline which is a 23 hour stay facility in Fife, acute services are bases at the Victoria Hospital in Kirkcaldy. The Breast Service is responsible for over 2500 new patient episodes each year and treats approximately 130 new cancers every year. A full range of breast procedures is offered by the unit, including reconstructive, oncoplastic procedures and symmetrising surgery. Free flaps are undertaken by the regional plastic surgery service.
The post holder will be expected to help in the administration of the unit and participate in operating lists, new patient clinics, follow-up clinics and emergencies where appropriate. The on-call commitment is currently two nights per fortnight.
Queen Margaret Hospitals
The Queen Margaret Hospital (QMH) is a 23 hour and short in-patient facility with 5 theatres and full out-patient department. One-stop and follow-up clinics are performed on site.
Pre assessment
Within VHK, there is a dedicated "Pre-Assessment Suite" which provides pre-operative assessment for all general surgical, orthopaedic, urology, oral, maxillofacial, breast and vascular specialties. It has been operational for more than 10 years and has a throughput of approximately 5500 patients per annum. The service has evolved into a comprehensive nurse lead assessment service providing the patient with vital personal contact, pre-operative information, instruction and follow up tests. Nurse practitioners perform full physical examination including heart and lung assessment, ECG interpretation and prescribing where necessary. The pre-assessment staff have an excellent working relationship with the cardiology department providing the patient with immediate cardiology opinion when necessary.

Whole time Specialty Doctor based at Queen Margaret Hospital, Dunfermline. This post entails provision of a comprehensive core Breast service to the Operational Division of NHS Fife with Consultant and Specialist colleagues. Previous experience in Breast Surgery is essential.

DUTIES AND RESPONSIBILITIES
Clinical
The key role of this new post will be to provide cover and support for the usual clinical activities of the Breast Service Team. These duties will include:
- Attendance at Outpatient Clinics, one stop new patient clinics and follow-up clinics
- Operating Sessions, for day case and short term inpatient procedures
- Inpatient ward rounds
- Attend weekly MDMs
- Provide administrative cover for the Breast Service, including dealing with GP referrals and managing long-term follow-up
- Out of hours cover will be required. This is currently two nights per fortnight to provide cover for post-operative cases and is shared with an Associate Specialist from NHS Lothian.
The post holder would be expected to have appropriate skills for management of the Elective Breast Surgical cases. Support would be available from more senior staff.
The post holder will be available for help and advice and to give opinions to a general medical inpatient or an A&E attendee as well as other hospital colleagues.
Administrative duties that would be part of the post include those associated with care of patients and sharing with colleagues the efficient organisation of the specialty.
The appointee will be expected to participate in all aspects of the Clinical Governance structures developed by the Trust to monitor, maintain and develop quality and effectiveness of care. As indicated above, this will include involvement in audit and appraisal. As part of the latter meeting requirements for continuing medical education and attention to continuing professional development will be required
Job Plan
The Job Plan is well established and consists of 10Pas. The level of participation in on-call will be agreed at job planning. Your job plan will be subject to review once a year by you, your colleagues in the Breast Service and the Lead Clinician/Clinical Director, as laid out in the terms & conditions of the new consultant contract.
In the event of any significant changes in the circumstances affecting this job plan, it is agreed that it will be renegotiated jointly in collaboration with consultant colleagues.

Part A: Fixed Programmed Activities (10 PA’s)
Weekly timetable of Fixed Programmed Activities* (i.e. regular scheduled NHS activities in accordance with paragraph 30B of the terms and conditions of service).
Each elective theatre session equates to a 5.0 hour Direct Clinical Care Session as defined by the Specialty Doctor contract which includes pre and postoperative care. We may be able to incorporate a specialty interest for a suitable candidate.
The job plan will be subject to review once a year by you and the Clinical Head for Breast, as laid out in the terms & conditions of the Specialty Doctor contract.
Appraisal/Revalidation
The successful candidate will participate in the appraisal and revalidation processes. You will participate in an annual appraisal. You will be allocated to an appraiser on appointment. The successful candidate will also attend an annual Mandatory Training update. Dates will be circulated by the Medical Director’s Office.
Teaching, Research and Audit
The successful candidate will be expected to take an active interest in the teaching of undergraduates and in the training of junior medical staff.
Candidates will be expected to encourage the audit and research activities of junior medical staff in addition to their own personal audit and research activities.
The post holder will be required to participate in multidisciplinary / multi professional meetings based at Queen Margaret Hospital

Continuing Medical Education
The Board supports and will require the successful candidate to participate in continuing medical education (CME). You are entitled to 30 days paid study leave within any 3-year period, with expenses for the purposes of CME.
Clinical Governance
The Planned has an active Clinical Governance programme, in which the junior staff are expected to participate. Clinically based research projects are frequently undertaken by junior staff with strong support from the Consultants and good secretarial support.
SPECIALTY DOCTOR IN BREAST
PERSON SPECIFICATION
CRITERIA / ESSENTIAL / DESIRABLEQualification / Candidate will have completed at least four years full time postgraduate training (or equivalent gained on a part-time basis) at least two of which should be in a specialty training programme or a fixed term specialty trainee in relevant specialty or shall have equivalent experience and competencies.
FRCS
Full registration with the GMC and hold a current licence to practise. / Postgraduate Breast qualification.
Experience / Three to four year’s training in Breast. / Experience of Breast in the UK
Knowledge / Knowledge and understanding commensurate with experienced SpR level.
Teaching / Proven ability to deliver high quality teaching. / Experience in teaching of undergraduates.
Research / Evidence of publications relating to Breast
Audit / Evidence of participation in audit activities
Management / Basic understanding of health service management structures.
Interest and Motivation / Genuine commitment to the Specialty.

TERMS AND CONDITIONS OF SERVICE
The post is covered by the Hospital Medical and Dental Staff and Doctors in Public Health and The Community Health Service (Scotland), Specialty Doctor Grade, Terms and Conditions of Service.
TYPE OF CONTRACT / PermanentGRADE AND SALARY / Specialty Doctor
£37,547 to £70,018 per annum pro rata.
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / Full Time 40 hours per week (pro rata to 40 hours for part time staff)
SUPERANNUATION / You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.
REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and should be discussed and agreed prior to appointment.

The region of Fife is bounded in the north by the Firth of Tay, in the east by the North Sea and in the South by the Firth of Forth. The region spans and area of 130,700 hectares and has a population of 360,000. Wide ranges of cultural activities are available in Fife and in the nearby cultural centres of Edinburgh and Glasgow. The main urban and leisure centres of central Scotland are closeby and there are excellent air, rail, and motorway links to the rest of the UK. Edinburgh and Glasgow Airports are within easy reach by road.
