POSITION TITLE:Community Facilitator

REPORTS TO:DIRECTOR OF NURSING

I.POSITION SUMMARY: Community Facilitator:

1. Gathers data, prepares agenda, attends and compiles minutes for a large number of LTC meetings.

2. Responsible for day-to-day staffing, payroll, and attendance tracking for LTC employees.

3. Maintains posted Nursing Hours as mandated by CMS

5. Performs clerical duties for LTC

5. Actively participates in Quality Improvement projects for LTC

lll. EDUCATION REQUIREMENTS/LICENSE/CERTIFICATE/REGISTRATION/EXPERIENCE:

1. High School graduate

2. Proficient Computer Skills

3. Experience in Long Term Care is preferred

IV.CUSTOMER FOCUSED EXPECTATIONS:

While performing functions of this position, the Administrative Secretary will understand the mission, vision and values of the Fulton County Medical Center and be able to articulate their application in Person Centered Care concept practiced in the neighborhoods on Long Term Care. He/she must be committed to transformation of the neighborhood into a Community where all team members and residents play an important role in the overall function and decision making of the neighborhood.

V.TYPICAL PHYSICAL DEMANDS (Subject to modification or accommodation as required)

1. Standing or walking 10-15% percent of the time, and sitting 85-90% of the time.

2. Occasional lifting and/or moving up to 25 pounds.

3. Hand-eye coordination, finger dexterity, functional visual ability and depth perception.

4. Reach with hands and arms, climb or balance, stoop or kneel.

5. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

6. Occasional exposure to outside weather conditions.

VII.Work Environment:

The Administrative Secretary’s work environment is usually quiet. However, exposure to noise, distractions and unpredictable behaviors will be experienced. Exposure to prolonged viewing of a computer terminal, eyestrain and repetitive motions will also be experienced.

ESSENTIAL FUNCTIONS

Gathers data, prepares agenda and compiles, distributes and maintains minutes of the LTC Nursing Meetings, Supervisors Meetings, Falls Committee Meetings, Psychoactive Medication Meetings, Wound Care Committee Meetings, Resident Care Committee Meetings and other meetings as assigned. / / /
Maintains accurate and confidential attendance records for all employees. / / /
Sorts and distributes internal and external mail correspondence for LTC. / / /
Receives incoming and outgoing telephone calls and greets visitors in a professional manner. / / /
Assists other departments with tasks as assigned, for example, scans for MDS Coordinator and LTC Social Services. / / /
Prepares reports for weekly, monthly and/or quarterly Quality Improvement Meetings. / / /
Once the schedule has been completed, serves as staffing coordinator responsible for day-to-day staffing, including working with staff and agencies to fill open holes and/or cover call-offs. / / /
Develops fliers and maintains attendance records for meetings, inservices, and special events for LTC. Schedules meeting places as appropriate for these activities.

Prepares LTC schedules and daily staffing sheets; enters these into computer and posts to intranet. / / /
Works with Infection Control Practitioner to notify families by mail that an infection has been reported. / / /
Tracks, inputs and verifies payroll for LTC nursing staff. / / /
Performs clerical duties for NHA and DON as requested. / / /
Maintains the LTC Policy and Procedure Manual in MCN; keeps files current with policy/procedure updates. / / /
