Position Overview
This position is responsible for the provision of assessment, intervention and support services to children, young people, their families and/or foster families who are subject to statutory intervention, including children placed in Pathways Fostering, Residential and Semi-Independent services.
The position holder is responsible to ensure that services provided are consistent with the mission, vision, and values of Churches of Christ Care and the Sanctuary Model of Care adopted within Care Pathways.

Key Result Areas
Key Result Area / Outcomes
Assessment / ·  Complete assessments that inform and make recommendations on most suitable placement and ongoing support options that would best meet the strengths and needs of the child or young person.
·  Assessments may include, but are not limited to: social assessment, strengths and needs assessment, ecomapping and Kinship and General Fostering assessment.
·  Refer assessments to other providers where they are beyond the professional scope or workload parameters of the position holder, and following consultation with the Service Manager.
·  Complete assessments within timeframes set by legislation, Service Agreements or in consultation with the Team Leader / Service Manager.
Residential/ Placement Support and Development / ·  Develop and deliver or source group work for children and young people living within Pathways Residential or Semi-Independent Living Services that are reflective of a common therapeutic need as identified by Case Manager / Residential Team Leader / Service Manager (Residential only)
·  Ongoing review and evaluation of group work programs to ensure that these continue to meet the current prevalent needs of a group of children and young people (Residential only).
·  Provide consultation to Case Manager / Team Leaders in the development of care planning for children and young people.
·  Develop therapeutic networks within the local community in order to engage external service providers as required.
·  Liaise and engage with significant people in the child or young person’s life in order to maintain and build positive relationships and connections for children and young people
Consultancy / ·  Liaise with relevant government offices and services as required.
·  Maintain open lines of communication with Team Leaders / Case Managers / Case Workers on the therapeutic services being provided to children and young people.
·  Research therapeutic resources within the wider community in order to facilitate the implementation of programs.
·  Provide consultancy in relation to specific child related issues and or group dynamics as requested by the Team Leader or Service Manager.
·  Maintain an up to date knowledge base of best practice therapeutic services within a child protection environment.
Recording and Reporting / ·  Prepare legible and legally defensible case notes and professional reports as required by Churches of Christ Care
·  Release of personal, private or confidential information is carried out with the consent of the Service Manager and in a way that is consistent with Churches of Christ Care’s processes and the Privacy Act.
·  Maintain data, statistics and other documentation as required under organisational
policy, legislation, or funding body / referral source requirements.
·  Seek internal review of professional reports from the Service Manager/ Program Support Manager – Therapeutic Care prior to their completion and forwarding to internal and external parties.
Service Development / ·  Raise the profile of the service through representing the service in the community and participating in networking and marketing activities.
·  Participate in ongoing planning, evaluation, and development of the service, including involvement in special events.
·  Work collaboratively with stakeholders to ensure best outcomes for children and young people in care are achieved by providing ongoing support, education and advocacy.
·  Promote effective working relationships with funding bodies, other agencies and key stakeholders, in consultation with Line Manager.
·  Participate in appropriate ongoing professional development activities.
Human Resource Management / ·  Mentor Case Workers / Case Managers / Youth workers, in consultation with the Service Manager / Team Leader.
·  Provide coaching and feedback to Case Workers / Case Managers / Youth Workers to further develop their practice skills.
·  Provide professional development opportunities for staff, in order to enhance therapeutic outcomes for children and young people accessing Pathways services
·  Provide day-to-day supervision of staff as required / requested by Service Manager
·  Participation in the recruitment and selection of service support staff / providers.
Customer Service / ·  Identify the needs of internal and external customers.
·  Develop strategies to meet customer needs.
·  Evaluate the delivery of customer service.
·  Use customer feedback and service delivery evaluation to improve services delivered.
·  Guide and work within the principles and commitments of Sanctuary Model of Care with staff and clients.
Quality Improvement / ·  Perform work in accordance with organisational policy, process control plans and work instructions.
·  Identify improvements to policy, process control plans, work instructions, or the work environment.
·  Communicate improvement opportunities to the relevant process owner/ other responsible party.
·  Conduct improvements to work processes in a manner that is consistent with the organisational policy and procedure.
Workplace Health and Safety / ·  Adhere to safe work practices at all times.
·  Report incidents and hazards immediately using established procedures.
·  Follow legislation and organisation policies and procedures in relation to workplace health and safety.
Other Duties
·  Attend service and divisional meetings, and team / professional development activities as required.
·  Undertake project work as directed by the Service Manager.
·  Represent the service in local forums.
·  Participate in facilitation and attendance at camps and outings with children and young people as required.
·  Provide ‘on-call’ services on a rotational basis or as required.
·  Other activities as required by the Service Manager.
Key Selection Criteria
KSC 1 / ·  Possession of a degree in Social Work, Psychology, Social Sciences, or other related discipline.
KSC 2 / ·  Demonstrated knowledge and experience of working with children and young people in a child protection context over a minimum period of two years
KSC 3 / ·  Demonstrated ability to provide leadership and mentoring to relevant staff.
KSC 4 / ·  Ability to operate windows applications at an intermediate level, including the following applications: Word, Excel, Outlook, and PowerPoint.
KSC 5 / Other requirements:
·  Posses Working with Children (Blue Card) - or the ability to acquire.
·  Possess Licensed Care Services Suitability Check - or the ability to acquire.
·  Possess a current Queensland driver’s license (or ability to acquire same within 1 month of employment).
·  Willingness to travel intrastate periodically.
KSC 6 / ·  Applied knowledge of legislation related to child protection and residential care and applied knowledge of what constitutes high quality service delivery to children and young people in care.
KSC 7 / ·  Highly developed skills in completing professional assessments and experience in developing and implementing therapeutic programs for children and young people.
KSC 8 / ·  Willing and able to work within a Christian context and Churches of Christ in Queensland’s mission and values.
