COMMUNITY OUTREACH ASSOCIATE JOB DESCRIPTION

General Summary: The Northeast Ohio Affiliate of Susan G. Komen is seeking a Community Outreach Associate (COA) to grow and manage Affiliate education and outreach programs and support grants and advocacy initiatives. This position requires an ability to build strong relationships with community organizations, volunteers, colleagues, and the healthcare community. The ideal candidate is committed to raising awareness about breast cancer, helps address breast health disparities, and measures breast health outcomes in the Northeast Ohio community.

Position Title: Community Outreach Associate

Position Classification: Exempt, full time (40 hours a week)
Reports to: Director of Mission

Primary Responsibilities

The primary responsibilities of the COA include: plan, implement, and evaluate the Affiliate’s community education and outreach programs, awareness events, and survivor support activities; support the community needs assessment; and provide assistance and support for other mission related programs.

Education and Outreach

·  Oversee, implement, and evaluate grant-funded Mahoning County education program with the goals of increasing organizational partnerships and educating community members, healthcare providers, and organizational stakeholders on breast health and breast cancer issues as determined by the Affiliate’s quadrennial community needs assessment.

·  Work collaboratively with the Education Committee to plan, develop, and implement Affiliate sponsored education programs within the Affiliate’s 22 county service area, including Breast Friends Forever, Minority Health Month events, Breast Health Awareness Month events, I Am The Cure, and the Affiliate speakers bureau.

·  Manage, coordinate, and support the Education Committee and its work to drive the education components of the Affiliate’s Mission Action Plan (MAP), including the facilitation of monthly committee meetings.

·  Develop and conduct regular training sessions with outreach volunteers (Komen Community Representatives), coordinate their participation in area events, and track their community impact. Establish sustainability measures for outreach volunteers post-training to ensure active engagement of all trained volunteers.

·  Coordinate outreach volunteer participation at local community events in conjunction with the Event and Marketing Associate.

·  Monitor educational materials inventory, fulfill all community educational materials requests received by the Affiliate, and prepare educational materials for Affiliate events as needed.

·  Create and maintain a database of relevant breast health and breast cancer programs and services offered in Northeast Ohio and communicate resources internally to staff and Board and externally to other agencies and members of the community.

·  Receive, respond to, and track community resource and patient navigation calls in a timely manner.

·  Assist in representing the Affiliate in area coalitions across the service area.

·  Develop ongoing, mutually beneficial relationships with key stakeholders in priority counties as identified by the Affiliate’s community needs assessment and communications plan.

·  Contribute, support, and develop a communication plan to keep volunteers, staff, Board, and community members up to date with breast health and breast cancer information, resources, and latest research. Act as an information resource for volunteers, staff and community.

·  In collaboration with the Director of Mission, conduct regular informational webinars and trainings on breast health and breast cancer topics for diverse audiences, including survivors, grantees, Board members, community members, and volunteers.

·  Monitor and update educational content of Affiliate website, including local and national resources, breast cancer screening events, breast cancer statistics, breast health educational messaging, and other relevant information.

Survivor Support

·  Plan and implement initiatives to support survivors based on survey results and the Affiliate’s community needs assessment.

·  Aid in the planning, implementation, and evaluation of the annual Breast Cancer Survivor Symposium in coordination with the Director of Mission.

·  Recommend and develop method to communicate latest developments in treatment to survivors via website content and/or other appropriate means.

·  Assist in the promotion of the Face 2 Face Network’s Young Survivor Support Group monthly meetings.

·  Develop ongoing relationships with survivors, hospital social workers/patient navigators, and survivor support groups to keep them engaged and connected with Affiliate activities.

Secondary Responsibilities

Community Needs Assessment

·  Actively participate on the ad-hoc needs assessment committee and assist the Director of Mission in coordinating all activities associated with the quadrennial community needs assessment, which evaluates breast health and breast cancer service needs in the Affiliate service area, and the bi-annual assessment update.

·  Assist in the development and delivery of trainings on the community needs assessment.

·  Support the creation of communications plan to disseminate needs assessment findings to the public, including legislative officials and sponsors.

Grants

·  Aid the Director of Mission in the administration of grants and maintenance of working relationships with grantees.

·  Act as a backup for and assist the Director of Mission in all grant related activities, including the Request for Applications (RFA); the Affiliate’s Grants eManagement System (GeMS); grant review, award, and oversight; grantee reporting; and grantee site visits.

Public Policy

·  Support the Director of Mission with the public policy efforts of the Affiliate, including the coordination of legislator attendance at Affiliate events.

Other:

·  Actively participate on the Affiliate’s Strategic Mission Committee.

·  Other activities deemed appropriate by the Director of Mission that will promote and enhance patient services throughout the Affiliate service area.

Position Requirements

·  B.A./B.S. in public health, health education, social work, political science, or other related discipline or a minimum of 3 years experience in related field.

·  Non-profit experience, Masters degree in public health, Certified Health Education Specialist (CHES), and knowledge of Spanish a plus.

·  Knowledge of key stakeholders and experience working in Mahoning County a plus.

·  Must have car, valid driver’s license, and automobile insurance.

·  Ability to lift at least 20 pounds.
