Position: Coordinator of Health Care Transformation

Normal Working Hours:8:30am to 5:00pm Monday through Friday

Reports To:Chief Strategy Officer

Responsibilities:

Operations Research20%

  • Responsible for tracking new standards and strategic plans that are developed by the Governor’s Office of Health Transformation, including OHT Goals such as modernizing Medicaid, streamlining health and human services, setting clear expectations for overall health system performance, integrating physical and behavioral health care, restructuring government funding systems around cost-containment, market exchange planning, and a permanent health and human services organizational structure.
  • Highest priority is understanding State Revisions around Health Homes, Medical Homes, and Accountable Care Organizations.
  • Develop fiscal models of Health Homes that can achieve state mandated outcomes. Attend statewide meetings of government entities and trade associations to give and receive input on Health Homes.
  • Assist Primary Care Physician and other personnel and related functions in data analysis for program improvement of central medical services.

Proposing New Models of Care: 20%

  • Participate in the development of new models for Health Home Services that meet State expectations.
  • Participate in the development of new models of Integrated Care.
  • Make recommendations for operating efficiencies.
  • Make recommendations that achieve better outcomes
  • Propose modifications of core services that will complement working models of Integrated Care.
  • Recommend staffing models that achieve results and create cost efficiencies.
  • Meet regularly with implementation teams to discuss joint recommendations for achieving system efficiencies and goals for new models of care.
  • Meet as required with other senior managers to promote the removal of obstacles that could dampen achievement of goals and better outcomes.

Program Growth and Expansion: 15%

  • Responsible for identifying opportunities for collaboration and developing proposals for collaboration in health-behavioral health collaboration.
  • Assist in the development of formal grants to foundations with the purpose of either developing new lines of services for The Village Network (e.g. consultation to child day care facilities) or expanding our services to new markets.

Private Insurance20%

  • With assistance of the CFO, develop relationships and learn the business contract requirements of private insurance carriers so that TVN can determine which insurance panels are feasible for TVN to work with.
  • In consultation with the CSO and the CFO, analyze current changing business requirements of our public funding sources to determine what shifts in service and organization are needed.
  • Analyze our lines of services from a clinical standpoint to determine our readiness to contract with private insurance carriers.
  • When requested by CEO and or CSO represent agency around new health transformation planning groups and service initiatives.

CQI10%

  • Work with CQI to assure we minimize risks while not avoiding legitimate challenges consist with our mission and business plans.
  • Work with Director of Quality Improvement to assure that new programs we develop meet COA standards and suggest new ways of efficiently meeting data acquisition needs and quality outcomes that are built around performance improvement.
  • When requested by CEO and or CSO represent agency around new health transformation planning groups and service initiatives.

Advance TVN Public Policy and Advocacy Priorities15%

  • Formulate, implement, and/or track legislative and other policy initiatives and strategies regarding specific issues of interest to TVN.
  • Develop and prepare analyses of current and proposed legislation and regulations affecting TVN as needed.
  • Support needs of the CEO, CSO and Trustees as they relate to policy activities, such as drafting letters, writing talking points and testimony and arranging and preparing for meetings

Qualifications:

  • Extensive software experience including Microsoft Office products.
  • Five years of relevant experience in closely related organization.
  • Advanced degree in business/social work.
  • Requires good organizational skills, excellent oral and written communication skills, and the ability to work productively with a wide variety of personalities.
  • Must exhibit an understanding of diverse health and human services and how they can contribute to insuring that clients lead as full and productive life as possible.
  • Ability to develop and deliver high quality presentations
  • Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
