Describe your research efforts to date and research plans for the future: In one to two pages, (1) what research projects you have worked on or are planning, (2) the extent to which you were the driving force behind those projects, and (3) how your work uniquely contributes to your field.

Curriculum Vitae: Include relevant details about your publications and presentations (authors, author order, venue)and whether submitted or accepted.

Evidence of Research Ability/Achievement: Please attach complete copies (with authorship) of published or submitted papers. Rejectedpapers/reports that will be submitted for publication in the near future, or substantive papersprepared for class may also be included. Be sure to explain your contribution to the work in yourstatement of purpose.

Other material: Please attach any other relevant material. If you have won competitive fellowships, represented yourlabs at conferences,reviewed papers for conferences and journals, sit on campus-wide committees, or represent yourpeers at faculty meetings please include this information.

Self-Evaluation (Attach to Annual Progress Evaluation Form):You are to complete a self-evaluation of your progress and performance during the year. The report will include your vita, current and future educational and professional goals, and a self-evaluation of personal and professional growth. The report should be submitted to your advisor prior to the end of March. Contact your advisor at the time you submit the report and arrange a meeting.

Advisor Evaluation (Attach to Annual Progress Evaluation Form):You and your advisor will meet to review the student report and the advisor/instructor evaluation forms. Together, you will discuss the year’s work, progress, and performance, and establish goals and objectives for the next year.
