Cheerleader Agreement and Release
As a participant in the St. Louis Rams Cheerleader tryouts and/or seminar/dance clinic in St. Louis and/or St. Louis County, Missouri, I agree to accept full responsibility for any risk or injury which I may incur as a result of attendance at or participation in these tryouts and/or seminar/dance clinic. I further agree, should I be selected as a St. Louis Rams Cheerleader, this Agreement and Release shall remain in full force and effect.
I, and my past, present and future agents, attorneys, representatives, heirs, executors, administrators, families, successors and assigns, forever release and discharge the St. Louis Rams, its shareholders, directors, officers, employees, agents, successors and assigns (the “Rams”), from any action, claim, demand, liability, injury, loss or damage whatsoever resulting from or arising out of my participation in connection with these events.
By my participation in these events, I expressly represent that I have adequate medical and liability insurance and agree that the Rams may rely on such representation.

Cheerleader Publicity Release
The undersigned does hereby grant the St. Louis Rams Partnership, doing business in the State of Missouri as the St. Louis Rams, and its successors and assigns (the “Rams”), the unrestricted right to use the undersigned’s name, likeness, or appearance on any football or similar cards, posters, calendars, photographs, video material, film material, electronic on-line services, or other similar promotional material in any form, content or medium in order to promote or market the Rams.
The undersigned does hereby expressly release and waive any demand, action, claim, license, royalty, or any other form of payment the undersigned, and his or her agents, representatives or assigns, may have based on claims of the undersigned as to the rights of privacy, publicity, notoriety or any other rights arising out of or relating to any use by the Rams of the undersigned’s name, likeness or appearance.
