The above named person(s) who is/are submitting an application to adopt a pet throughPAWS for Irving Animals, LLC, hereinafter referred to as “PAWS,” agrees to be bound by the following terms and conditions: (Please type/sign your initials for each to confirm)

Type Initials / I understand that adopting a pet is a commitment to a lifetime of love and care for the pet. This may be for 10 or more years.
Type Initials / I understand the importance of regular vaccinations and heartworm preventatives. I agree to ensure that my new pet receives regular vaccinations and heartworm preventatives.
Type Initials / I understand the importance of micro-chipping my new pet. If my pet is not already micro-chipped, I will get him/her micro-chipped at the first veterinarian appointment.
Type Initials / I understand that the long-term health of my pet cannot be guaranteed. I understand that animals that appear healthy may be infected and not show symptoms until after I have brought them into my care. I understand that a veterinary check-up is recommended for my new pet within the first ten days of ownership.

Type Initials / If for any reason I should not be able to maintain ownership of my new pet at any point in his/her life, I will not take it to an animal shelter facility that euthanizes. I will not “dump” the pet. I will contact PAWS for Irving Animals LLC at or via Facebook at facebook.com/PAWSforIrvingAnimals immediately for assistance in re-homing my pet.
Type Initials / Adopting a pet is not without risk. I understand that PAWS is not responsible for:
oDamage to my personal property caused by adopted pets.
oIllness or injury to my own animals caused by adopted pets.
oPersonal injury caused by adopted pets.
oCosts incurred as a result of unauthorized veterinary treatment of adopted pets.
Type Initials / I understand that the adoption fee is $150 for dogs older than 12 months and $200 for dogs under 12 months of age.. This can be paid via check to PAWS for Irving Animals LLC or via PayPal to . The fee must be received before the adopter can take possession of the pet. The fee includes: DHLLP, bordatella, de-wormer, rabies vaccine, spay/neuter and microchip. (Rabies, microchip and spay/neuter might be coordinated for future date based on age of the pet at adoption.)
Type Initials / (For Puppies under 4 months only) I understand that my pet is currently not spayed/neutered. PAWS will authorize an appointment for me to get my new pet spayed or neutered and my adoption is not complete until the surgery has been completed.

INDEMNITY RELEASE

I, Enter Name here , shall not hold PAWS for Irving Animals LLC or Irving Animal Services liable, or responsible for, and PAWS for Irving Animals LLC and Irving Animal Services shall be saved and held harmless from and against any and all claims and damages of every kind, for injury to any person or persons, and for damage to or loss of property arising out of or attributed to, directly or indirectly, the operations or performance of the above named potential adopter under this agreement, including claims and damages arising in whole or in part from the negligence of PAWS for Irving Animals LLC or Irving Animal Services.
