The 4th Annual Centralia Rotary Project Foundation sponsored PHATE Golf Tournament (Prostate Health Awareness Thru Education) has been scheduled for Friday, August 26, 2016 at Newaukum Valley Golf Course. Chehalis, Washington. The purpose of the golf tournament is to raise funds that will provide a free to the public educational forum on prostate health/cancer and gynecological cancer by our partner Providence Regional Cancer System.

The education forum held last year on October 1, 2015 at the Great Wolf Lodge was our most successful event to date attracting over 300 people from throughout the area. The 2015 golf tournament was also our largest to date with a total of 106 golfers. With the continued help of your sponsorship we hope to attract an even larger audience for this year’s event tentatively scheduled in the month of September.

Please consider one of the sponsorship levels listed below:

1.  Major sponsorship - $1000. Your name will be included in media advertising for the event. Included is a foursome for the golf tournament. Includes a table for advertising purposes at the tournament lunch and at the public education forum.

2.  Golf hole sponsorship - $300 per hole. Your name and company logo on a sign at one of the golf holes on the tournament date. You may have an advertising display at your hole.

3.  Golf cart sponsorship - $200 per cart. Your name on a golf cart on the tournament date.

4.  Team sponsor - $300 per foursome. ($75.00 per player)

5.  Golf tournament prizes: We are also in need of items you may be able to provide to serve as raffle prizes for the golf tournament. Prize sponsors that donate items valued at the Major sponsorship level will also be provided a foursome for the tournament.
