Dance Pass Permission Form - 2016-2017
Dear Parents:
The Plaza Vista Student Council will be sponsoring three dances this year for Plaza Vista middle school students ONLY. Plaza Vista Annual Dance Passes are sold at Jump to Success at a cost of $20.00 for all three dances. (No dance passes will be sold the day of the dance!) This pass provides entrance into all three dances, (except the 8th grade Promotion Dance) at a savings of $10.00. If your child purchases an Annual Dance Pass, no individual ticket purchase or individual permission form is needed for each of the dances.
Please refer to the following behavior expectations and information:
DANCE BEHAVIOR EXPECTATIONS:
1. All students must present their ID cards at the door of the dance. An ASB sticker will be placed on the back of your child’s
photo ID card.
2. Once students arrive at the dance, they must remain until 8:00 P.M., unless they are picked up personally by a parent or
guardian. NO RE-ENTRY is allowed.
3. Students may not use and/or purchase another student’s dance ticket.
4. Students who are absent from school on the day of a dance will not be allowed to attend the dance.
5. Plaza Vista’s Student Behavior expectations are in effect at all dances. This includes dress code NO GUM.
6. Inappropriate behavior at a dance (before, during or after) will result in the loss of any or all dance privileges.
7. Students with “U’s” in Citizenship from any previous Progress Report or Report Card will not be allowed to attend
the upcoming dance.
8. Students who have been suspended during a trimester of a dance may not attend. There will be no refunds issued.
* 9. Should your child want to walk home from a dance, you must contact Mrs. Perini in writing. Should your child want to
go home with a friend, you must notify Mrs. Perini giving her the friend’s name, their parent’s name & their phone
number for each dance.
PRICE: $20.00 for an Annual Dance Pass (3 dances, excluding the 8th grade promotion dance). Student Photo ID cards must have
an “ASB” sticker on the card and MUST be presented at each dance. Please check your calendars carefully. No refunds will be given should a student decide he/she does not want to attend, or if an athletic event takes place the day of
the dance. A refund will be given only if a student is ill the day of the dance.
Refreshments (pizza, water bottles, and soft drinks) will be available for a small fee.
TIME: The dances are from 6:00 – 8:00 P.M. Students are expected to be picked up by a parent or guardian immediately
after the dances are over, or the student will lose the privilege of attending the next dance.
