Dear Parents or Guardians:
Please complete the following information regarding the placement of your child for the 2015-2016 school year. We will use your comments along with the input of your child’s current teacher to determine placement. Among our considerations will be:
- Teaching styles/student learning styles
- Maintaining an appropriate balance in each classroom
- Class size (to ensure all classes begin the year with an equal number)
Teacher request forms for students entering grades K – 6 will be available April 20th–May 1st. Request forms are available at your child’s current campus or online at All teacher request forms must be returned to your child’s current campus office. For specific teacher requests to be considered, eight names must be listed on the request forms. Teacher request forms picked up at your child’s current campus have a list of the next grade level’s teachers on the back. If printing your request form from the district website, please refer to campus webpages for a list of teachers at each grade level.

For specific teacher requests to be considered, eight names must be listed below. (NOTE: They will not be considered in any particular order when making placement decisions. Every effort will be made to honor your requests, as long as we can be consistent with our placement criteria listed above.)

*IMPORTANT INFORMATION:
- Forms are available online and in campus offices.
- Deadline for submitting teacher request form for students enteringgradesK-6 is Wednesday, May 1, 2014.
- Forms submitted for these grade levels after this date will be considered but may not be honored due to classroom space limitations.
- Forms will not be considered that list names of other students.
- Forms will not be considered if less than eight teachers’ names are listed.
- RETURN ALL FORMS, K-6TH GRADES, TO YOUR CHILD’S CURRENT CAMPUS OFFICE.
- Forms for new students entering Kindergarten for the 2015-16 school year will be available at Kindergarten Round-Up.
- FORMS FOR STUDENTS NOT YET REGISTERED WILL NOT BE ACCEPTED.
