Program Design Forms

Program Design Form

The Program Design Form will be used by the PFT to create exercise programs for clients/FF’s. The form is designed so a comprehensive whole body program can be developed for each client/FF.

Following the Program Design Form is a list of exercises a PFT may use to develop an exercise program. Exercises for each exercise component of the Program Design Form are listed for clients/FF’s of different fitness levels. PFT’s should use these exercise as a starting point when developing an exercise program for a client/FF.

Strength Training Form

The Strength Training Form is designed for those clients/FF’s who are prescribed a more extensive strength training routine. This form is best used when split routines are designed to facilitate a higher volume of training for specific body parts.

Cardiovascular Training Form

The PFT can use this form to design a more comprehensive cardiovascular program that is focused on a specific goal. For example, a client/FF wishing to improve their 10K time may want to include intervals and hill running on specific days of the week. This form can be used to list the specifics for each cardio session.
