Job purpose:

Providing quality support toindividuals in the community/supported living environment with all aspects of life toenable them to live as independently as possible.

Main responsibilities and duties

  • Provide emotional and practical support, advocacy and information to the individuals we support.
  • Support Individuals to take any prescribed medicine, complying with all medication policies including storage, administering and record keeping.
  • Support individuals that may have challenging behaviour.
  • Enable Individuals to achieve independence as far as possible in all areas of their life by providingappropriate information and support.
  • Support Individuals to identify and choose their support needs.
  • Assist the individuals we support to present themselves as valued members of the local community.
  • Create opportunities for social, leisure activities with Individuals and support attendance where needed.
  • Ensure that the rights of the people we support are always protected and respected.
  • Provide a safe, comfortable and supportive home for the individuals we support.
  • Support Individuals to actively contribute to the running of their own house.
  • Report any maintenance and repair tasks in the houseto the Service Manager.
  • Provide First Aid services at the house/service when necessary and if trained to do so.
  • Assist in health and safety assessments, follow health and safety procedures and participate in fire drills and audits.
  • Assist in devising, implementing and reviewing care plans.
  • Participate in rotas, which may include day, evening, weekend and bank holiday working and sleeping-in duties at night.
  • Liaise with parents and other professionals, e.g. general practitioners, dentists, etc.
  • Keep records and participate in administrative tasks.
  • Assist in writing, receiving and updating of individual care plans in conjunction with the Manager.
  • Assist and participate in the wide range of administrative task necessary for the smooth running of the house/service.
  • Participate in the opportunities provided for training and development.
  • Participate in regular supervision and an annual appraisal to receive support, identify training needs, evaluate work performance and assess professional development.
  • Attend staff development programmes, training courses, team meetings and workshops as required.
  • Work within, and promote, the policies and procedures of Personalised 4 Autism.
  • Maintain confidentiality at all times about Individuals, staff and the company as a whole.

Undertake any other duties consistent with the overall purpose of the post as directed by the Manager.

Required knowledge and skills

Good communication skills.

Ability to empathise with individuals we support

Experience of care

Ability to be flexible to the needs of the service.

Full UK driving licence driver with Business cover and access to a car

NVQ or equivalent qualification in Care
