PERRYMEMORIALHOSPITAL

EMERGENCY OPERATIONS PLAN

SUBJECT:Code Silver (Active Shooter/Hostage)

PURPOSE:To provide guidanceto staff members and guests who are confronted by a person (s) wielding a weapon, or one who has taken hostages within PerryMemorialHospital or its properties.

POLICY:It is the policy of PerryMemorialHospital to rely on police assistance for the resolution of any weapons / active shooter / hostage incident.

PROCEDURE I.Response to the incident is as follows:

A.The first employee to identify a weapon / hostage situation shall:

1.Secure the immediate area if possible by removing all patients and personnel.

2.Secure doors if possible to isolate the situation.

3.Contact the Switchboard to report the CODE SILVER. Name the location of the incident, type of incident (gun,knife,other) and the perpetratorif known.

B.The Switchboard Operator shall:

1.Contact 9-1-1 to report the situation including the locationandtype of incidentwithin the hospital, and the perpetrator (if known)along with any other pertinent information.

2.Page the “CODE SILVER and the location.

3.Notify the Administrator- on- Call, Security Officer, Safety Officer and House Supervisor.

C.The Administrator-on-Call will initiate Incident Command.

D.Security staff on scene will:

1.Observe / assess the scene and report any and all pertinent information to Incident Command including:

a.Cause or type of problem (if known)

b.Number of perpetrators and hostages

c.Description of weapons if known

2.Seal off the area restricting access completely until police arrive.

3.Evacuate all persons remaining in surrounding areas.

ONCE POLICE ARE ON THE SCENE AT THE HOSPITAL, THEY ARE IN CHARGE OF THE INCIDENT.

PROCEDURE II.Staff / Patient Safety

A.If possible, leave the area and take patients/visitors with you.

B.Report your observations to Incident Command.

C.If you cannot leave the area safely, try to put a barrier between you and the shooter.

1.Close / lock and/or barricade your office doors.

2.Turn off the lights

3.Get under / behind heavy furniture.

4.If in a patient room, barricade the door and go into the bathroom and hold or block the door shut.

5.Try to remain calm.

D.If you are not in close proximity to the incident:

1.Close area doors and shelter-in-place.

2.If police order evacuation of the building, proceed to the nearest staircase and exit the building.

3.Meet in the designated area. DO not leave the premises. Department heads are responsible to ensure that all staff are out of the building and report to incident command.

4.Stay out of hallways and common areas (waiting rooms etc.)

5.House Supervisors do NOT report to the area of the incident. Establish contact with Incident Command and follow their instructions.

E.The “All Clear” can only be authorized by Law Enforcement Officers.

1.The “All Clear” will be announced over the telepage system

2.The Public Information Officer is responsible for media briefings.

3.All staff, patients and family members involved in the incident will be offered options for counseling.

4.The Disaster Coordinator and Incident Commander will coordinate the Post-Incident briefing / hotwash to gather information for the After Action Report.
