District Nurse Transformation Board

Time commitment: / Meetings are typically held on a monthly basis.
Venue: / Meetings are usually held at Bridgwater Community Hospital (free parking)

The work of the District Nurse Transformation Board:
The District Nursing Transformation Board is responsible for identifying and implementing key areas of change in relation to quality of care and staff and patient safety
Roles and Responsibilities:
The role of the Patient Representative is to ensure that the views of and impact on patients, public, carers and families are takeninto consideration during relevant discussions, in particular, wheneverdecisionsthat affect or impact on patient care or carers are made.
The Patient Representative is expected tocontribute to both the service development and decision making processes.
The Patient Representative is a valued member of the Board and will have the full support of the Workstream Lead or Chair, who will provide ongoing support.
Patient Representatives will work to:
Contribute, both in meetings and between meetings, in a constructive manner, offering ideas and opinions which reflect the voice of patients, public, carersand their families;
Where applicable, Patient Representatives should seek views and feedback from the group they are representing.
Provide a commitment to the Group, attending as many meetings as they are able.
To adhere to a Code of Conduct.
To sign a confidentiality agreement.
We will ensure that you have any necessaryinformation formeetingsandensureyour voice is heard and your viewpoints considered.
This workstream’sPatient Representatives will need:
  • Good communication skills
  • Experience of the District Nursing service, for example through being a carer for a patient that has used the service, or by using the service yourself.
