Job Purpose: /
  1. Strengths based assessment, triage and case work with people who have multiple and complex needs
  2. Partnership working to reduce rough sleeping and improve outcomes for service users

  4. Developing and co-ordinating joint working between the outreach team and all three large day centres in Westminster.

Salary and Scale: / Salary: £26,865 - £31,140 pa (including London Weighting)
Scale: 6/ SO1 Points 26-31
Contract: / Permanent Full-time Positionworking within rota with shifts usually being conducted between 8.30am and 4.30pm Monday to Friday.

MAIN TASKS

1Strengths based assessment, triage and case work with people who have multiple and complex needs

  1. Conduct assessments for outreach service users who come into the Day Centre using a strengths based approach and contribute towards ensuring the appropriate case allocation, signposting or onward referral.
  2. Developing use of strengths based assessment and ongoing support work in Outreach and sharing across the team(s).
  3. Strengths based recovery case work with people who have complex needs

2.Partnership working to reduce rough sleeping and improve outcomes for service users

  1. To lead on effective communication between the Day Centre team and the CAS so that our clients are assessed and advised how to come off the streets as quickly as possible
  2. Facilitate supported reconnections for our clients to the services and areas most appropriate for them.
  3. Liaise as required with other statutory and voluntary organisations including collecting and providing information and attending external meetings
  4. Take a multi-agency problem solving approach working in collaboration in regard to case conferences and joint work with other agencies involved with the client (such as drug and alcohol treatment providers, local day centres, health professionals and hostel providers).
  5. Refer people to specialist services, such as substance use, welfare rights, mediation services, medical and legal services as appropriate.
  6. Support the development and implementation of a CSTM Migrants Policy, ensuring we are able to meet the needs of homeless migrants where possible

3. Testing new methods of engaging with clients who have multiple and complex needs including personalisation and co-production

  • Identifying clients on the Outreach caseload to test out new methods of engagement
  • Use personalised budgets and other tools to engage clients and improve outcomes
  • Investigating best practice across the homelessness, mental health, drug and alcohol recovery and criminal justice sectors and innovating practice across the Outreach services.
  • Evaluate impact and disseminate learning across the team
  • Contribute towards the developmentand expansion of the co-production (client involvement in service design, delivery evaluation) across outreach services, engaging those with lived experience of homelessness in supporting the recovery of others who use services and contributing towards the design, delivery and evaluation of services.

4.Developing and co-ordinating joint working between the outreach team and all three large day

centres in Westminster.

  1. Developing operational working relationships between the Outreach team and the day Centres, evaluating success and feeding back
  2. Participating in an initiative across CSTM, West London Mission (Seymour Place day Centre) and The Passage Day Centre to consistently link day centres and outreach in the borough.

5.Monitoring and evaluation of service delivery to demonstrate impact

  1. Participate in the development of a culture of evidence collection, review and continuous improvement
  2. Data input and case recording to support monitoring, evaluation and continuous improvement.
  3. Dissemination of learning across the organisation and wider sector.
  4. Support client involvement in, and feedback on the service.

6.Peer Mentoring and Client Involvement

  1. Liaise with peer mentoring agencies (e.g. Groundswell) to bring volunteers to the streets to engage clients with complex needs.
  2. Liaise with the Client Involvement Co-ordinator to get feedback and input from clients about the Outreach service.
  3. Contribute towards the development of co-production and peer mentoring across outreach services and wider CSTM provision.

7.General

  1. Implement The Connection at St Martin’s Equality Policy and all other policies in all areas of work.
  2. Ensure that Health and Safety requirements are complied with.
  3. Attend internal and external training courses as appropriate.
  4. Assist in the promotion of the work of The Connection at St Martin’s to its visitors and funders.

PERSON SPECIFICATION

Innovation and Intervention Worker (Outreach)

This person specification sets out the essential experience and abilities needed by the successful candidate for this post. Please bear these points in mind when completing your application form, as these requirements will be taken into account at both the shortlisting and interviewing stages.

1. Knowledge
An understanding of the causes of homelessness and the needs of homeless people including demonstrable knowledge of relevant legislation, policy and practice, e.g. Rough Sleeping Strategies, Mental Health Act, Housing Act, Enforcement Orders, Central and Local Government Strategies
Knowledge of relevant external agencies, resources and facilities that would enhance both thinking and action on client involvement within CSTM;
Knowledge of current client involvement good practice and approaches such as personalisation, co-production and strengths based approaches;
Knowledge of welfare rights available to the client group including EEA Migrants (desirable)
2. Experience
At least 3 years full-time experience (or equivalent) of face to face work with vulnerable people within a charity setting;
Strong strengths based case work experience with people who have complex needs (in particular drug and alcohol recovery but mental health, criminal justice and homelessness are all relevant);
Experience of creating positive partnership working across agencies
Experience of working with peer mentors
Experience of co-production, personalisation and psychologically informed environment approaches.
3. Abilities and Skills
An enabling approach, with the ability to work effectively with people who have multiple and complex needs;
Ability to lead others at the same level through change, without having specific management responsibility;
The ability to prepare and present written reports, and to communicate information effectively, both verbally and in writing, to clients and other workers;
The ability to manage time and prioritise workload effectively;
The ability and willingness to work flexible hours where required;
The ability to exercise initiative and work under pressure;
The ability to respond appropriately and effectively when faced with challenging or violent behavior and the personal stress associated with this
Strong skills in data input and review including good IT skills and an interest ininnovating the use of technology to support clients on the street.
Language skills particularly Polish, Romanian or Spanish (desirable)
