Consent Form

You are invited to participate in a usability study conducted by students of UC Berkeley. As part of a class project, we are designing an iPhone application for skateboarders who wish to improve their skateboarding skills, and we hope to use the feedback from this study to improve our current design so we can create an application that users will find convenient and helpful. No deception is involved, and no risk or discomfort is anticipated as a result of your participation.

Participation in the study typically takes less than an hour. Participants begin by watching a researcher perform a simple task on our prototype, after which they will be given three different tasks to perform on the prototype. As participants perform the tasks, we will observe and record (in written and audio forms) participants' reactions to various parts of our design as well as various other measures of performance. Participants are encouraged to express their thoughts aloud as they use the prototype and provide feedback about their experiences after each scenario.

All responses are treated as confidential, and in no case will responses from individual participants be identified. Rather, all data will be pooled and used by the research team for the sole purpose of the class project. Please be aware that the records may be published on a class website that can be accessed by the general public. No material will be used without your explicit consent.

Participation is voluntary, refusal to take part in the study involves no penalty or loss of benefits to which participants are otherwise entitled, and participants may withdraw from the study at any time without penalty or loss of benefits to which they are otherwise entitled.
