PARTICIPANT WAIVER AND RELEASE OF LIABILITY

ASSUMPTION OF RISK AND INDEMNITY AGREEMENT

PARTICIPANTS IN THE NORTHERN CALIFORNIA IRISH WOLFHOUND CLUB (“NCIWC”) SAN FRANCISCO

ST. PATRICK’S DAY PARADE (“Activities”) ARE REQUIRED TO ASSUME ALL RISK OF PARTICIPATION IN THE ACTIVITIES BY ACKNOWLEDGING THIS GENERAL RELEASE AGREEMENT.

I, the undersigned Participant,certify that I am the actual owner, duly authorized agent, family member or invited guest of the owner or agent of the dog participating in today’s NCIWC activities. Further, Participant on behalf of himself/herself and on behalf of Participant’s personal representatives, assigns, heirs, executors, hereby fully and forever releases, waives, discharges and covenants not to sue the NCIWC, its officers, directors, chairperson, representatives, and members; and all municipal agencies whose property and/or personnel are used, including volunteers; and all other companies; organizations; or individuals; related to the Activities (collectively “Releases”) from all liability to Participant and his/her personal representatives, assigns, heirs and executors, for all loss(es) or damage(s) and any and all claims or demands therefore, on account of injury to Participant or property or resulting in the death of the undersigned, whether caused by the active or passive negligence of all or any of the Releases or otherwise, in connection with his/her participation in the Activities.

Said Releases shall extend to any claim for loss or injury alleged to have been caused directly or indirectly to Participant’s dog, or to any person or thing by the actions of said dog, while in or upon the walk, or grounds near the start and finish of the event. Participant personally assumes all responsibility for the actions of said dog, as well as Participant’s authorized agent, family member or invitee. These Releases shall extend to any claim for loss of the dog by disappearance, theft or injury caused or alleged to be caused by the negligence of the NCIWC, or any of the aforementioned parties; including any loss or damage to any personal property, which shall include Participant’s parked vehicle, while attending the NCIWC Activities.

Further, Participant certifies the said dog is well socialized and under Participant’s control, and poses no known hazard to any person animal or private/public property it may encounter at the Activities.

Participant represents and warrants that he/she is in good physical condition and is able to safely participate in the Activities. Participant is fully aware of the risks and hazards inherent in participating in the Activities and hereby elects to voluntarily participate in the Activities, knowing the risks associated with the Activities.

By signing below, the Participant acknowledges that he/she has read, understood and agrees to this Participant Waiver and Release of Liability Assumption of Risk and Indemnity Agreement.

If the Participant is not yet 18 years of age, this Agreement must also be signed by the parent(s) or guardian(s) of the Participant.
