Part Time Practice Nurse

17 hours per week

We are looking to recruit a part time Practice Nurse to join our friendly 2 site Practice. Some holiday cover will also be required.

Experience of Chronic Disease Management is preferred but training in chronic disease will be supported. Salary is dependent on skills and experience.

2.  JOB PURPOSE
To assess need, plan and implement programmes of nursing care for a group of patients within a General Practice setting.
To Provide Nurse Led Clinics within the GP practice.
3. ORGANISATIONAL POSITION
Professional & operational accountability to GPs
4. SCOPE AND RANGE
· To assess patients, plan and implement care and provide advice within the General
Practice Primary care setting
· To supervise students as required, e.g. nursing students.
· To receive patients with undifferentiated and undiagnosed problems, and make an
assessment of their healthcare needs based on highly developed nursing
knowledge and skills including physical examination.
· Contribute to training ethos within the practice.
5. MAIN DUTIES/RESPONSIBILITIES
· Organise and provide nurse led clinics (e.g. cervical cytology) ensuring that
procedures are carried out and updated in accordance with local/national policy/
procedures.
· Organise and provide primary prevention programmes, e.g. Well Person, Weight
Management.
· Develop and manage chronic disease management programmes in accordance
with National guidelines, e.g. CHD, Diabetes and Asthma.
· Provide specialist care, education and ongoing support for patients and carers.
· Draw up protocols and care pathways for chronic disease management and update
as required.
· Maintain stock control.
· Participate in ongoing audit and implementing change within the practice.
· Operate at all times within agreed Health & Safety Policy as set nationally and locally.
· Implement policies within determined parameters.
· Be actively involved in the administration of insulin initiation therapy for Type 2
Diabetes by providing ongoing counselling and education in accordance with NHS
Dumfries & Galloway professional practice policy.
· Maintain monthly quality assurance control tests on blood glucose monitoring
equipment.
6.  SYSTEMS AND EQUIPMENT
· Use electronic information system to access/input patient information for clinical
purposes and audit.
· Use equipment required to carry out any procedure detailed in Procedure Manual,
where the individual is deemed proficient.
7. DECISIONS AND JUDGEMENTS
· Be accountable for own professional actions: not directly supervised.
· Work autonomously in clinic setting: referring to GP, GP referring back.
· Make judgements on problems requiring investigation, analysis and change in
treatment of management of condition.
· Organise own workload.
8. COMMUNICATIONS AND RELATIONSHIPS
· Communicate sensitive/specialist condition related information to patients, relatives:
empathy and reassurance required.
· Operate appointment and recall system.
· Maintain concise, up to date clinical records in agreed formats.
· Liaise with members of the Primary Health Care Team.
· Refer direct to other services e.g. podiatry, smoking matters, healthy bowel and
bladder services.
· Regular meetings with GPs/PM to evaluate and change practice where required.
9. PHYSICAL/MENTAL DEMANDS OF THE JOB
· The postholder will be expected to exhibit the appropriate degree of dexterity
whilst performing clinical procedures, e.g. venepuncture & surgical intervention.
· Occasional light/moderate effort required.
· Moving and handling operations following guidelines laid down by NHS
Dumfries & Galloway.
· Concentration for many areas of the work including taking smear tests and
listening to patients symptoms.
10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB
· Impart complex and sometimes upsetting information to patients and their carers,
whilst exhibiting a level of empathy and reassurance.
· Regular exposure to unpleasant conditions, e.g. body fluids, aggressive behaviour,
physical or verbal.
· Assisting GPs in management of change associated with the new GMS contract.
· Keeping up with continual changes in technology in limited time span.
11. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB
· 1st Level registered nurse, RGN RMN
· Diploma level specialist training in Chronic Disease: Diabetes, CHD,Asthma & COPD
· Breast and Cervical Screening qualification.
