Part-time Nurse, Contract for 6 months with potential for renewal

Job Share, 35 hours per 2 weeks

We are looking to expand our nursing team. The successful candidate as a member of our inter-professional team will provide nursing support for the primary care clinic supporting clients of Country Roads Community Health Centre.The inter-professional team participates in the development, implementation, monitoring and evaluation of primary health care services for individuals, families and the community.

Key Areas of Responsibility:

  • Clinical leadership including but not limited to: assessment, treatment, chronic disease prevention and management, health education and counseling
  • Working to the full scope of practice
  • Care coordination and system navigation for complex clients
  • Continuity of care includes home visits and provision of extended hours as required
  • Working in a manner that reflects an understanding of the determinants of health
  • Uses quality improvement practices consistent with evidence-based best practice guidelines
  • Collaboration with other members of our inter-professional team including family physicians, nurse practitioners,community pharmacist, social workers, registered nurses, respiratory therapists, dietitian and community health promoters.

Qualifications:

  1. Undergraduate degree or diploma in nursing from a recognized university/college
  2. Licensed as a Registered Nurse, or Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO)
  3. 2 years minimum primary health care experience in a community setting
  4. Training and experience in quality improvement
  5. Demonstrated commitment to inter-professional team environment
  6. Experience with electronic health records
  7. Experience in program development, implementation and evaluation an asset
  8. Valid driver’s license
