Dear 6th Grade and Guys Club ChorusParents:

In connection with the study of Choral Music,your child will have the opportunity to take a field trip to Southern Star Festival/Six Flags which is located in Atlanta, GA. We can be reached at the following telephone: 404-915-7673. The trip is being sponsored by Vanessa Edwards. The event will take place on Saturday, April 22nd, 2017. The schedule for the event is attached to this form. Please detach the schedule and keep it for your records. We will be traveling by Charter Busand will have at least 10adult chaperones. The expected ratio is 8students for every adult.
The cost to attend this trip is $90 for 6th grade students, $75 for 7th/8th Guys club members, and $50 for adult chaperones. Please complete the bottom of this form with your choice for student involvement, attach your paymentif necessary, and return it no later than February 10th, 2017.

I, ______give permission for my childto be driven by a charter bus and agree to release, indemnify, and hold harmless the Fulton County School System, its Board of Education, and its employees from any liability for damages or injuries in the event of an accident caused by such driver. I also understand that one meal is included in the cost of the event, and that I need to supplement any further meal costs on the day of the event.

I do not give my permission for my child to attend the above event.

______I am interested in volunteering to chaperone this event. I understand that the cost for chaperones is $50, which includes bus transportation, a park ticket, and a meal pass.
