Purpose:Pan de Vida Foundation was founded to meet the basic human needs of the communities of Queen Creek and San Tan Valley.

Award Components:Each year Pan de Vida Foundation will select four[4] recipients, each of whom will receive a $500 scholarship that rewards students engaged in community service, have a 2.0 gpa or higher, and are currently enrolled in a high school located in the Queen Creek and San Tan Valley area.

Award Impact:The one time assistance payout of $1000, Pan de Vida hopes to boost the confidence of students from low income families who need a jump start to their education after high school.

Award Criteria:

1. Applicant must be a registered student at a high school located within the Queen Creek and San Tan Valley area.

2. Applicant must be a junior or senior level student as defined through credit hours at theirinstitution.

3. Applicant must show a proven record of community service.

4. Applicant must have a minimum, cumulative GPA of 2.0 or higher on a 4.0 scale.

5. Applicant must be a U.S. citizen or permanent resident with a permanent resident IDcard or current passport.2

Application Process:

Applicant must submit the following items:

1. Completed application form (if handwritten, please print legibly).

2. Letter of intent addressed to the Pan de Vida Foundation stating their intent to apply for the scholarship.

3. Two (2) character references in the form of a letter of recommendation.

4. A copy of their most recent transcript with cumulative grade point average.

5. Personal Essay. In your essay, please answer the question on the enclosed essayform.

How has community service made a meaningful impact in your education?[300 words or more]

6. Applicant must agree to a personal interview with the selection committee in person, upon request as neccessary.

7. Applicant must agree to a media interview (ex. Newspaper) along with a photo if awarded to be published.

8. Applicant must agree to release all rights to photos to be used for Pan de Vida Scholarship collateral.

9. Letter of acceptance from school for the following school year.

10. Applicant must present U.S. birth certificate or proof of residency.

11. Applicant must present income records for parents or legal guardians for the past year.

Deadlinefor the application isApril 30th of every calendar year beginning with 2012. Applications postmarked after this date will not be considered. No emailed application packets, incomplete applications will not be considered.

Announcement of Scholarship Recipients:May of every calendar year beginning with 2012

Special Provision:Scholarship funds will be issued directly to the recipient’s campus and may not be used to pay back college loans of any kind.
