Thank you for your enquiry dated 5th February 2016where you requested information regarding Solent NHS Trust’s relationship with Hampshire Health Record.
I confirm that we hold the information as below.
- Please could you tell me whether your organisation has commenced extracting and uploading personal confidential data about patients/ clients to the HHR?
Yes
- If not commenced already, do you have plans to commence feeds into the HHR at any time between now and 31 March 2017?
N/A
If the answer to both of the above is no, then thank you very much, ignore the remainder of this request, and please consider my request closed. However, if your organisation has already commenced uploading, or is planning to before March 31st 2017, then please can you provide me with the following?
- Any and all fair processing information (posters/ leaflets/ hand-outs) made available or planning to be made available, to patient/ clients informing them of this new use of their data.
The HHR is a commissioning led initiative and therefore they are responsible for all communications. For more information on the HHR please see:
However Solent NHS Trust does inform patients that their information is shared with neighbouring NHS organisations and GP’s. Please see embedded documents below available to patients.
- Any and all policies on the implementation of HHR feeds by your organisation
The implementation of HHR was introduced and implemented prior to Solent NHS Trust’s establishment, therefore this information should be sought from our Commissioners
- Southampton CCG
- Portsmouth CCG
- Hampshire CCG
- The types/ nature of information that you will be extracting and uploading to the HHR

- The opt-out form (or process) available to patients to express dissent to this type of processing of their data, and so clock uploading of all their personal confidential data to the HHR by your organisation (in line with the DPA and NHS Constitution)
To opt out of the HHR patients are advised to contact their GP.
