Faith Formation 2016 - 2017 Registration Form

Welcome to Faith Formation SY2016-2017. We are excited that you made this choice to continue your child’s development in his/her faith. This year is going to be filled with lots of faith driving fun, activities, and fellowship. Not only is it important that your child continue his/her journey of faith through the program, but we encourage the assistance from you parents. You do not need toknow all the answers but be willing to commit yourself to living, learning and teaching thevalues of Christian life and the Catholic faith especially at home We ask that parents and guardians assist in the program by making sure your children get to the class on time as well as encouraging the participation at the Sunday masses.

Faith Formation fee offsets the cost of textbooks and activity books, supplies/materials, catechist training, etc. necessary for the parish to offer a faith formation program.

Payment for registration fees must accompany the form before your child will be placed in a class. If for financial reasons you are unable to make a payment at this time, we will arrange for you to meet with Father Marvin to discuss your financial situation. Only registered members of our parish will be allowed to register for our Faith Formation Program. A registered member attends weekly mass at Our Lady of the Blessed Sacrament Church and uses their envelopes to financially support the parish.

I understand that my child’s attendance is required for the Faith Formation Program and that failure to attend classes may result in his/her not receiving a Sacrament.
