The American Case Management Association’s (ACMA) annual National Case Management Week (NCMW) will take place Oct. 13-19. The theme for this year’s celebration, “Broadening the Continuum of Care,” emphasizes the critical role of hospital case managers in managing health care transitions across multiple providers throughout the continuum of care while also balancing the responsibility to serve as advocate for both the patient and the health care system.

On Sept. 27, the United States Senate unanimously passed S. Res. 214, a resolution declaring the week of Oct. 13-19, 2013, as "National Case Management Week," to recognize the value of case management in improving health care outcomes for patients.

"As healthcare continues its transformation, case management has become even more essential to the care process. National Case Management Week is an opportunity to recognize the dedication, compassion, and outcomes achieved by case managers. This Senate resolution will provide exposure to the critical practice of case management, and help better inform the public on the role of case managers in representing and advocating for patients, families and their organization," said ACMA President Sharon Mass, Ph.D., LCSW, ACM.

ACMA created NCMW to recognize the important contributions of case management professionals in health care delivery systems across the nation. These clinically competent professionals are responsible for addressing key challenges faced by health care delivery systems including caring for uninsured patients, reducing readmissions and costs, adjusting to governmental and private insurance changes, resolving psychosocial issues, and increasing the quality of care their patients receive.
