Please ensure you read this form in conjunction with the Yogie Awards 2017 Nomination Package. This supporting document provides a full description of the criteria for each award, which should be addressed in your nomination.

This form can be used to nominate an Organisation, Service, Program or Project for more than one award category. At Question 3, please only complete the relevant part for each category you are nominating for.

Be aware that members of the judging panel may not have prior knowledge of the Organisation, Service, Program or Project you are nominating. Please provide enough detail to ensure the panel has a fair understanding of the great work they are accomplishing.

Testimonies from participants can be attached separately, or included in Question 3. These are not included in the word count, and can be in the form of a supporting letter from a young person or staff member, screenshots of social media feedback regarding an event or program, quotes from participant feedback or surveys, photos of an event or program in action, or media articles.

What makes the nominee outstanding, innovative, unique or special? Who has been involved? Why has it worked? How did it achieve its aims? What are you most proud or happy with?

3a. Outstanding Achievement in Youth Participation Award

For example: How have they demonstrated an outstanding commitment to developing meaningful partnerships with young people? How have they demonstrated best practice principles for youth participation that is meaningful, appropriate, effective and beneficial to young people? How have young people involved had their views and contributions heard and actioned? How have young people benefited? Max. 300 words

3b. Innovation in Service Delivery Award

For example: How have they thought and acted “outside the square’ to deliver services to young people? Have they identified new ideas, programs and/or practices that have been useful in working with young people, the community sector, government, business and/or the broader community? How have they converted knowledge and ideas into improved ways of working? Have they collaborated with other groups? How has this impacted and been valued by young people and the community? Max. 300 words

3c. Excellence in Organisational Practice

For example: How have they demonstrated excellence in developing or their staff? What organisational practices have they improved, or provide ‘stand out’ support for their staff? Have they collaborated with other agencies to increase staff capability? Have they introduced governance systems which better support staff or the organisation? What is the impact this has had on young people? Max. 300 words

3d. Excellence in Research and Evaluation Award

For example: How have they demonstrated their commitment to conducting best practice research? And/or how have they demonstrated their commitment to on-going quality improvement through evaluation? How have young people’s voices been heard in the research or evaluation? How will this research impact young people? Max. 300 words

4. Do you have any supporting documents, testimonials from young people or colleagues, or any other additional information to support your nomination?

Supporting documentation is not required, however, you may wish to provide quotes from participant feedback, a supporting statement from someone who works with the nominee, or media articles or photos.
