This Operations Memo addresses revisions to the Intervention Assignment (IA) and the Discontinued (DC) Forms. In addition, the new Interventionist Characteristics (IC) and Intervention Delivery Assessment (DA) Forms with corresponding Q x Q’s are introduced.
Please use the following pages of the Forms and Q x Q’s as replacements for the current pages in the Core Manual of Operations, Volumes II and III.

Intervention Assignment (IA) Form
ISSUE: The IA Form was missing an intervention assignment category for the Philadelphia site.
SOLUTION: “Philadelphia No Exploratory Booster Program”, listed as category 19, was added as an intervention assignment to the IA Form. This information is relevant only to the Philadelphia site.
Discontinued (DC) Form
ISSUE: Protocol for asking question 6 on the DC Form (Within the past week, has (CR) experienced any behavior or memory problems?) if the care recipient has passed away.
SOLUTION: Question 6 is not appropriate to ask if the care recipient has passed away. In this case, the question should be skipped during the interview and a “not applicable” recorded. This question should be answered in all other circumstances.
Interventionist Characteristics (IC) Form
ISSUE: Capturing the characteristics of the interventionists/contact persons involved with REACH study participants.
SOLUTION: The IC Form and corresponding Q x Q’s have been developed to collect this information.
Intervention Delivery Assessment (DA Form)
ISSUE: Capturing the structural aspects of the REACH interventions.
SOLUTION: The DA Form and corresponding Q x Q’s have been developed to gather this information. Each site should first complete an DA Form for each intended contact within each intervention at their site and send this information to Joy Schweers at the Coordinating Center by Wednesday, February 11, 1998. The site should then fill out a form for each actual contact as it occurs.
