  1. On the reverse side of this page, list the Department/Number, Course Title, and Credit Hours for each course you might take at the host institution as described in the host college catalog.
  2. Meet with your UARC or academic advisor. With the help of the advisor, verify how each course will be accepted (e.g., major requirement, major elective, elective, core requirement). Make sure your academic advisor signs this form.
  3. Meet with your Department Chair(s) for upper level, major, and minor courses. Additionally, meet with Julie Meyer, , the Transfer Evaluator in the Admissions Office (Dempsey 135) to discuss lower level and elective courses. Make sure all appropriate personnel sign this form.
  4. Bring this form to the NSE coordinator (Dempsey 202). The NSE Coordinator will make copies for you, Julie, and the host institution (which the NSE Coordinator will send to). The original will be filed in OIE.

Courses transferring from a semester calendar to a quarter calendar will generally earn one third more value than quarter credit hours.
