1Justification Detail:

Add field to support Copy to: Physician address (Submitted by Quest Diagnostics in proposal 6xx), Copy to Physician Fax, and Copy to Physician Lab Account number.

Copy to Physician in OBR-28 assumes that the individual sending the order has a well know identifier that will allow the report to be properly routed to the individual (example in the US an NPI known as National Provider Identifier. This is not always the case, often the only information available is the copy to Physician Name and address, typically the office address where they typically practice or the office closest to the Ordering Provider. Additional fieldsare proposed to be added that allows for copy to address, FAX and Lab Account number. The address field has already been submitted by another organization and therefore this request is intended to extend on that proposal by adding the FAX number and Lab Account number.

OBR-51 can be empty even if there is data in OBR-28 (Result Copy to), however if an address must be provided for a single entry in OBR-28, then there must be an appropriate entry in OBR-51 even if the repeat is empty.

Example: Only the second repeat in OBR-28 needs the address, then the following would appear in OBR-51
