OFFICIANT: PLEASE READ INSTRUCTIONS PRIOR TO FILLING OUT LICENSE

IT IS YOUR RESPONSIBILITY to return the License and Record of Marriage to the County Clerk within 5 calendar days after the ceremony – DO NOT give the couple the paperwork for filing (ORS 432.173). FAILURE to submit the License and Record of Marriage within 5 calendar days may result in a Class A misdemeanor civil penalty under ORS 432.995 and ORS 106.990.

The couple cannot prove their marriage until we have recorded the License and Record of Marriage into the public record, so it is important that the original (state’s copy) of the document be sent to us within 5 calendar days of the marriage ceremony.

This license contains two dates. A marriage ceremony may not be performed before the effective date or after the expiration date. It is your responsibility as the person performing the ceremony to complete blocks 29 through 33 of the License and Record of Marriage:

BLOCK 30a:The date and time you performed the ceremony using thefull name of the month DO NOT use a number or an abbreviation to designate the month.

BLOCK 30b:Where the ceremony was held (city, town or location)

BLOCK 30c:County where ceremony was held

BLOCK 31a:Your signaturein permanent black ink.

BLOCK 31b:Your official title (minister, judge, etc.)

BLOCK 31c:Your printed name, address (including zip code) and phone number

BLOCK 31d:Name, address (PO Box is not accepted), and phone number of your authorizing religious congregation or organization

BLOCK 32:Printed name of witness (not a signature please)

BLOCK 33:Printed name of witness (not a signature please)

Since the license will become a permanent state and county record, please make sure all information is correct and then fill in the blocks using BLACK INK or TYPEWRITER … all information must be LEGIBLE!

After the keepsake Certificate of Marriage and the License and Record of Marriage are filled out and signed (the parties should sign their name as it appears on the License and Record of Marriage), give the ornate embossed keepsake certificate to the couple and return the original License and Record of Marriage to the County Clerk’s Office. Retain the officiant’s copy of the License for your record.

NOTE: Crook County interprets ORS 106.150 “in the presence of” to mean Party A, Party B, officiant, and 2 witnesses must be in the same room, without the use of any electronic means such as skype or telephone, throughout the ceremony
