Assessment Activities Reporting Template

2016-2017

Directions: Use this template to document your assessment activities for the year. This template temporarily replaces assessment documentation in the University wide assessment management system pending the acquisition and implementation of the new assessment management system. Answers should provide enough details on your assessment plan and the results gathered throughout the year. The completed template should be ready, reviewed by your supervisor and sent directly to the Office of Institutional Analysis and Effectiveness by June 1, 2015.

Assessment Unit / IT Enterprise Application Services
Mission Statement / Supports high-quality administrative support by providing the technical framework and enterprise application services that advance the university's strategic goals; promote effective and efficient work processes; and support information decision making and institutional reporting in advancing undergraduate and graduate education.
Unit goals / 1.  Provide technical support and administration for secure and stable database management platforms, and a reliable suite of enterprise software applications in support of the University’s core business functions (Banner ERP, interfaces, EagleNet, Reporting, etc.).
2.  Develop and support a CORE (Comprehensive, Operational data-based, Reliable, Extensible) Reporting Strategy for the University.
3.  Improve overall efficiency, throughput, customer support and service delivery as provided by the EAS Department.
Goals assessed / Identify goals assessed during the year. [Each program must assess at least two goals.]
Assessment Methods / Provide details on the assessment methods used for each unit goals assessed during the year. [E.g. For Goal 1, students will complete a survey at the end of the year. The survey will be assessed four qualities of student experiences (see attachment). For Goal 2, attendance and spending on programs will be monitored. For Goals 3, performance will be compared against State recommended standards.]
Success Criteria / Preferred outcomes from the assessment results [E.g. At least 80% of students will be satisfied with their experience.]
Results / Details of the assessment results. [This may include trends, pattern, strengths and weaknesses, and any other observations that support the assessment process. Provide as much details as possible. Also cite and reference any supporting documents.]
Supporting Documents / Documents that support the assessment plan and results. [E.g. rubrics, assessment data, homework samples, artifacts, etc.]
