POLICY TITLE: PROGRAM EVALUATION

POLICY STATEMENT

All programs administered by Home and Community Support Services of Grey-Bruce will be monitored on an ongoing basis in order to evaluate the program's operation and its impact on participants. The results of this evaluation process will be used to modify programs to meet clients' changing needs.

PROCEDURE

  1. The Executive Director will ensure that the following data is gathered on a regular basis(no less than every two years): client profiles, program utilization rates, sources of referrals, reasons for discharges and per unit cost. This data, combined with other forms of feedback will form the basis for program evaluation. Staff may use the Evaluation Checklist (Form ADM4) to assist in compiling information.
  1. Ongoing feedback will be obtained via formal and informal means from staff, volunteers, participants and or their caregivers. Any organization with an active role to play in the management of the client's plan of service should also be approached for feedback.
  1. Any evaluation should focus on the extent to which the program:
  • is achieving the goals and objectives of the program.
  • is able to meet the needs of the clients;
  • integrates with the local long term care service system;
  • complies with government service agreements, standards, policies and legislation;
  • complies with the policies and protocols of the agency;
  • utilizes resources (staff, funds, facilities) in the most effective, efficient manner.
  1. In conducting any formal evaluation there will be a written plan which includes:
  • the timetable for initiating and completing the evaluation;
  • the parties to be involved
  • the areas that will be addressed; and
  • the methods to be used in conducting the evaluation
  1. A written report of formal program evaluation and findings will be brought to the Executive Committee of the Board of Directors.
