The successful candidates will be responsible for providing shift by shift clinical leadership to the nursing team, ensuring the delivery of high quality, person centred care. The post holder will work closely with the Senior Charge Nurse in promoting an integrated approach to care, ensuring safe and effective clinical practice and managing and developing the performance of the team. The individual will be responsible for helping ensure high professional standards within the nursing team, with demonstration of core NHS values being integral to achieving this.
Experience/
Qualifications / Applicants are required to be RMN or RNLD qualified with 4 years experience at Band 5 and above within the forensic environment.

2. JOB PURPOSE
To support the Senior Charge Nurse and co work with the other Team Leader(s) in the running of the ward and Hub, by;- Taking charge of the ward and providing Clinical Leadership to the nursing team during a span of duty.
- To develop, maintain and deliver high quality care in a secure environment, through the assessment, planning, implementation and evaluation of patient care processes, for defined group of patients (i.e. all patients within ward)
- To keep abreast of trends and developments in nursing and to participate in the planning, development and evaluation of new ideas and methods for the constant improvement of the services provided.
- To lead, contribute and influence an innovative and progressive attitude to the care of the patients.
- To support and facilitate the work of other disciplineswithin the wards and off ward areas; to supervise junior staff and be able to teach other nursing and non-nursing staff.
- This role requires an individual with the ability to provide patient care and to set standards within clinical areas. There is a need for knowledge and understanding of patient clinical issues and promotion of choice, respect and dignity.
3. DIMENSIONS
With direction from the Senior Charge Nurse;
- Lead and participate in the clinical, operational and managerial functions of the ward as required of the post.
- Lead and contribute to the management and continuing development of a team of nurses at band 5 and below.
- Ensuring the supervision of registered nurses, nursing assistants, student nurses and associated health professionals.
- Ensuring staff skill mix is appropriately maintained over the period by compilation of duty roster.
- Responsible for the Health, Safety and Security of the ward/ department when on duty.
- When on duty responsible for the direct care of patients on the ward.
- Staff are expected to be flexible and work within all patient care areas of the hospital
- Staff are expected to contribute to delivering the NHS Quality Ambitions of safe, effective and person centred care.

5. ROLE OF DEPARTMENT
The Nursing Directorate works collaboratively with the other clinical professions to safely rehabilitate patients detained under the mental health legislation, and in doing so provides the highest quality of nursing services for dangerous and potentially dangerous patients who are mentally ill or who have a learning disability, and who may exhibit severely challenging behaviours.6. KEY RESULT AREAS
Working with Patients:- To lead and participate directly in the assessment, planning, implementation and evaluation of patient care on the ward.
- To coordinate and direct nursing interventions to a group of patients and monitor via the usage of developed specialist knowledge the quality of nursing care provided by a group of staff,
- To ensure that appropriate documentation is used in respect of individualised patient care and that safekeeping of active and archive documentation is maintained within the ward/department,
- To direct unqualified staff providing day to day care of patients in procedures such as bathing, nutrition and general hygiene.
- To ensure that junior staff managesafely patients' clothing, possessions and valuables.
- To escort patients on outings.
- To escort patients between wards and departments.
- To take charge of and participate in patients recreation and social activities as a part of their continuing care at the hospital
- To contribute to the assessment of the individual and individual plans in response to the patient’s physical, social, spiritual, cultural and psychological needs, monitoring same to ensure the achievement and maintenance of the highest quality of patient care.
- To afford a high priority to health promotion and improvement initiatives for patients,
- To regularly supervise the practice of registered and unregistered junior staff.
- To empathise with patients and assist them to express their thoughts, feelings and needs.
- To actively promote and participate in a range of therapeutic nursing strategies with the patient group.
- To take an active role in promoting multi-disciplinary working.
- To deputise for the Senior Charge Nurse.
- To assist the Senior Charge Nurse in the effective implementation of the hospital’s attendance management policy,
- To ensure PDP completionfor a designated staff group in consultation with the Senior Charge Nurse.
- To ensure effective, efficient and economic utilisation of resources.
- To be an authorised signatory for an agreed list of non stock items
- To assist the Senior Charge Nurse in the auditing of practice within the ward.
- To assume additional responsibilities as and when delegated by the Senior Charge Nurse/Lead Nurse.
- To act as a group leader/co-coordinator for a defined set of nursing care plans.
- To contribute towards the development of the clinical agenda of the ward by evaluating and proposing change to ward practice.
- To participate in the induction process and ensure that all staff have a suitably focused induction and orientation to the ward, security and nursing policies.
- When on duty, to take charge of the ward and in doing so: -
- Report to the Senior Charge Nurse on all relevant clinical and administrative issues
- Ensure that at all times the ward remains physically and emotionally secure
- Ensure thorough and comprehensive hand-over meetings between shifts, incorporating individual patient reports and completion of all relevant documentation.
Working with a professional nursing framework
- To set a professional example in attitude, conduct and practice as laid down in the NMC Code.
- To utilise appropriate documentation for the assessment, planning and implementation of patients' treatments/care and to participate in clinical team meetings.
- To implement and follow clinical policies and procedures in the delivery of care within own area and proposes changes as required.
- To prepare, give and receive written and verbal reports, reporting conditions of patients, to more senior nurses and /or medical staff.
- To ensure safe custody, storage and administration of medications in accordance with Hospital Policies and NMC guidelines.
- To participate in nursing research in relation to forensic mental health nursing.
- To participate in clinical audit.
- To be a mentor for student nurses, and to work closely with the Practice Education Facilitator in ensuring an excellent learning environment.
- To maintain an up to date knowledge base of current clinical issues.
- To actively participate in the clinical supervision process as supervisor and supervisee.
- To remain updated in developments in clinical practice by participating in relevant post registration education.
- To maintain an up to date knowledge of current clinical issues and legalities.
- To work within the provisions of the State Hospital Health and Safety and Welfare policy,
- To ensure that at all times a safe and secure environment is maintained through involvement in audit and acting on concerns,
- To supervise junior staff whilst they are carrying out specific directed nursing duties such as laundry, menu planning, cleaning body fluid/ waste spillages and general ward housekeeping duties,
- To ensure that hospital policies and procedures are understood and adhered to at all times,
- To be familiar with and understand how to use the hospital radio system and personal alarm system,
- To report to the Senior Charge Nurse any matter relating to the safety of the environment or any breach of security
- To act at all times in a responsible and safe manner whilst in the wards and encourage a warm and welcoming environment for patients, visitors and other staff.
- Ensure application of policy and practice as it relates to infection control.
- To work within a multidisciplinary team and be involved in both verbal and written communication as part of the continuous review process.
- To liaise with parents and carers providing information re patient care that is in line with service guidelines.
- To observe the need for professionalism in both confidential and cultural issues.
- To maintain a level of communication that is clear, open and honest and without prejudice towards colleagues or others.

7a. EQUIPMENT AND MACHINERY
The post holder will be required to use the following equipment for the reasons stated:- Moving and Handling equipment to assist patient mobility (hoists, wheelchairs, shower aids),
- Medical equipment to record vital signs and administer treatments (Blood Pressure monitor, thermometer, syringes etc).
7b. SYSTEMS
- Information will be required to be input to various IT systems on behalf of patients (for example Electronic Patient Record, DATIX).
- Information will be required to be input to various IT systems in respect of staff management for example; that the SSTS and rostering system is accurate and that the planned rosters make effective use of the budgeted nursing resource, this involves inputting information generated by others.
- DATIX to input details of accidents and injuries.
- In addition, e mail messaging is the normal method of rapid communication within the StateHospital; the post holder will be expected to be familiar with this.
- Manual/ paper based record systems recording information about patients will also require to have information input to by the post holder.
8. ASSIGNMENT AND REVIEW OF WORK
- The post holder will assign work to the nursing team during their span of duty.
- Post holders are expected to work with a great deal of autonomy and be able to use their own judgement.
- Post holders work with minimal supervision within a management framework.
9. DECISIONS AND JUDGEMENTS
- The post holder will be frequently required to make judgements involving a range of facts or situations, which require the analysis or comparison of a range of options.
- Whilst working with patients who exhibit behaviours that challenge and at the same time have complex needs due enduring mental illness and a history of unmanageable behaviour in other settings the post holder will frequently need to make judgements as to the most appropriate intervention. This will be reviewed later, in consultation with the Senior Charge Nurse, if the situation requires it.
- The post holder will be required to make decisions regarding the operational management of the ward/area, and, supervise others in their decision making processes.
10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB
- To continually maintain a positive attitude towards the care of patients detained in The State Hospital
- To ensure human rights principles (lawful, legitimate and proportionate) is applied into all practice whilst employed by the hospital.
11. COMMUNICATIONS AND RELATIONSHIPS
- The post holder is required to receive and provide complex clinical information to patients, colleagues, and visitors. For example discussing sexually offending behaviour, index offence work in a hostile and antagonistic atmosphere. This requires the use of highly developed interpersonal and communications skills
- The post holder is required to receive and provide operational information to patients, colleagues, and visitors for example :- on security issues, change in practice i.e. zero tolerance, no smoking ,change of ward
- Many of the patients within the hospital have difficulty with communication; the post holder will be expected to be competent in both verbal and non-verbal communication methods.
- They will be expected to communicate effectively with patients, staff,professional visitors i.e. solicitors/external doctors/mental health officers, relatives and carers
12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB
High secure Mental Health environment that cares for mentally disordered offenders who are assessed as posing a grave and immediate danger to themselves and the public. This involves being the subject of searching and security procedures, responsibility for keys and working in a restrictive, locked environment.
Exposure to physical and verbal aggressive behaviour of patients and carers.
Physical demands:
- There is a frequent requirement to exert intense physical effort throughout the period of work, for example running from a standing start, up to 800 metres, to emergencies, working with other staff during the management of violence and aggressive incidents, both within own area and on other wards and departments,
- There is a requirement to respond to serious and emergency incidents and support efforts to manage them, both within own area and on other wards and departments.
- There is a requirement to undertake prescribed mandatory training, for example, management of violence and aggression (PMVA), health and safety, fire safety.
- Occasional driving of vehicles outside the hospital with a patient and other staff as passengers
- There is a need for constant awareness of security procedures including searches, patient movement restrictions, special observations levels and emergency procedures.
- The post holder is subject to periods of sustained and intense concentration, whilst having the knowledge, skills and experience to constantly switch from one task to another to meet the operational demands of a high secure caring environment.
- Caring for patients with severe forensic histories whilst maintaining a warm and empathic non judgemental attitude. These patients have severely challenging and or manipulative behaviours.
- Faced on a daily basis with distressing and emotional circumstances due to patients extremely challenging behaviour.
- There may be frequent exposure to verbal and physical aggressive behaviour from patients and carers, and the risk of being assaulted by a patient, the post holder is required to carry a personal attack alarm.
- There will be exposure to passive smoke,
- There will be a requirement to work in areas with exposure to highly unpleasant working conditions. These will include body fluids, soiled linen and patient areas where smoking takes place.
13. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB
- Registration on the NMC single professional register in respect of Mental Health Nursing and/or Learning Disability Nursing.
- Specialist in depth knowledge in a high secure forensic mental health setting or similar environment, where the post holder has a unique knowledge underpinned by practical experience...
- Experience of a similar working environment as Band 5 Staff Nurse for a minimum of 4 years is essential.
- Effective communicator, with a responsible and caring attitude who is motivated and committed to work with patients with a serious mental illness, or a learning disability.
