Declaration by the Father/Guardian:

I declare that in the event of admission of my son/daughter/ward to the Abdus Salam School of Mathematical Sciences (ASSMS), Lahore:

  1. I (father/guardian) will consider my self responsible for the payment of School/University fee and other dues, etc.
  2. I will also be responsible for making up any loss damage caused by the applicant to the School /University property during the course of his/her stay in the School / University.
  3. If, during the course of his/her studies, the applicant takes part in any political activity or becomes a member of a political or students organization, I will be personally responsible for it and affirm that the Vice Chancellor/Director General will be within his rights to strike the applicant’s name off the School/ UniversityRolls.
  4. I affirm that the Vice Chancellor/Director General can debar a student from appearing in the Examination for lecture shortage etc.
  5. The Vice Chancellor/Director General is authorized to strike a student’s name off the School / UniversityRolls as a punishment for the infringement of University Rules and Discipline.
  6. The documents submitted by my son/ward for admission are proved incorrect at any stage whatever or found to be fake or contain a mis-statement or error which may affect his/her merit, I will be held responsible and the School/University will have the authority to cancel his/her admission and to undertake disciplinary/Legal action, against him/her.

Declaration by the Applicant:

  1. I Pledge to fully abide by the Rules and Regulations of the ASSMS, GC University, and that I will not take part in political activity of any kind.
  2. I pledge that, I will not keep in my possession weapons of any kind whether licensed or unlicensed.
  3. I affirm that I have not taken admission in any other University or institutions; I also affirm that I will not take admission in any other Institution during my stay in the ASSMS, GC University. If proved, otherwise at any stage or time, my admission may be cancelled forthwith and all my dues paid to the ASSMS, GC University forfeited to the University.
  4. I affirm that I was neither expelled/rusticated by any Institution at any times, nor was ever any disciplinary action taken against me, nor did I ever take part in any unlawful activity.
  5. I affirm that if at any stage the documents submitted by me for admission are proved forged, fake or carry a mis-statement or clerical error which may affect my merit, I shall be held responsible and that the University will be within its rights to cancel my admission and to take the necessary action against me.
  6. I undertake not to join or support any students Union or group or political organization during my stay in the ASSMS, GC University.
  7. If I do so the ASSMS, GC University authorities would have the right to cancel my admission.

Note:

  1. The DG/Vice Chancellor has the authority to withdraw from the rolls of the ASSMS, GC University names of students who are irregular in their classes or guilty of breach of discipline.
  2. Students who fall short of lectures will not be allowed to appear in the final examination.
  3. Students who are admitted to the ASSMS, GC University are expected to make good use of the educational opportunity given to them. If they do not appear or fail to qualify the University mid-term examination, they shall not be allowed to appear in the final examination.
