The Caseworker is that member of patient care operations whose primary function is to assist in providing direct casework services to the patient and family within the organization.Their direct role is assessing needs and coordinates the appropriate services in the adult medicine, or OB/GYN departement.
Responsibilities:
  • Assist the patient/family in identifying appropriate community resources to help address their needs.
  • Assists in completing a variety of applications for patient/family assistance and with gathering required documentation with various community agencies.
  • Assists with transportation needs for meetings with community agency personnel and, if necessary, transports the patient/family representative to meetings with community agencies.
  • Evaluates patients who are admitted on a charity basis on the program in order to establish if they meet criteria for any funding or resources offered by various community agencies.
  • Evaluates all patients considered for admission as Medicaid pending through completing a financial assessment.
  • The Caseworker is responsible for making sure communication is provided to the local state office upon admission, and that it’s submitted within the proper time frames.
  • Colaborates internally and externally with providers, speciality clinics and other community partners
  • Other duties as assigned
Job Requirements:
  • Reliable transportation with adequate insurance coverage for driver and passenger
  • Extensive knowledge of community resources within the specific community of Norwalk, Ct.
  • Knowledge of local, county, applicable state and federal assistance programs
  • Ability to become proficient in company software programs
Qualifications:
  • Bachelor's Degree in Social Work or a related field from an accredited school
  • Two years experience in a healthcare environment
  • Good oral, written and organizational skills
  • Ability to work within an interdisciplinary team structure
  • Bilingual prefered
