Title of Post:Nursing Auxiliary – Hospice at Home

Location:Up to 25 mile radius from home

Reports to:Hospice at Home Co-ordinator

Responsible to:Head of Adult Hospice Community Services

Role

To provide support and respite care for patients and families in their own home. To deliver high quality nursing care to patients with palliative care needs by implementing and evaluating individualised care plans.

Clinical Practice

  • Implement and evaluate the care planned by the district nursing team.
  • Assist in the care of patients, including personal hygiene and sanitary needs independently or in partnership with the family.
  • Help, move, position and mobilise patients safely using the appropriate equipment.
  • To work alongside patients and carers, whilst recognising their need for privacy.
  • To maintain a safe environment for self, patients and carers at all times.
  • Observe general condition ofpatients in line with the care plan and record any changes in the patient held records.
  • Record in writing, all work completed in patient held records.
  • Report regularly to the District Nursing Sister and/or Hospice Nurse Specialist through agreed lines of communication.
  • Attend, as necessary, staff meetings, study sessions and other events where appropriate.
  • Acquire clinical competence as defined by the needs of the service.
  • Recognise own professional limitations and work within his/her scope of practice.
  • Promote and maintain good communication with patients, families and carers.
  • Liaise with the Hospice at Home Co-ordinator on a regular basis.
  • Be familiar with the philosophy of Northern Ireland Hospice and aware at all times of the role in representing the Northern Ireland Hospice.

Education, Research and Professional Development

  • Undertake and complete the required induction programme.
  • Complete all mandatory training within a specified timeframe.
  • Attend staff meetings on a regular basis.
  • Attend study days as required.
  • Participate in annual review.

General Responsibilities

  • Liaise closely along agreed lines of communication with co-ordinator with regard to availability for work.
  • Submit availability as per operational policy.
  • Submit monthly returns within the specified timeframe.
  • Adhere to Northern Ireland Hospice policies and procedures.
  • Participate in the investigation of incidents, accidents and complaints.
  • Ensures the economic use of supplies and resources.

Job Specification

Band:Based on Band 3 (£16,955 pa pro-rata/ £8.67 per hour)

Essential Criteria

  • A minimum of 2 GCEs/GCSE’s (including English Language) Grades A – C (or equivalent qualifications) and at least one year’s experience of working in a healthcare setting

OR

Two years’ experience working as a carer in a healthcare or community setting.

  • Good interpersonaland communication skills.
  • Access to transport to enable the requirements of the post to be fulfilled.
  • Ability to work unsupervised within a patient’s home.
  • Willingness to undertake education/training programme.
  • Willingness to be flexible with hours as dictated by needs of the service.
  • Willingness to travel within a 25 mile radius of own home.

Desirable Criteria

  • Experience of caring for palliative care patients.
  • Experience working in a community health care setting.
  • Ability to demonstrate an understanding of the role of Hospice in the community.
