TEAM NOMINATION FORM
We wish to make application to compete in fixtures held by Noosa District Netball Assoc. Inc. on Saturdayat McKinnon Dr Tewantin. We agree to pay the required fees and abide by theConstitution and By-Laws of the Association and the Rules of Play of All Australia NetballAssoc Inc.
NOTE: If you have a 10 or under player who is planning to play competitive netball please note the child’s name, DOB and team on the form. The child will need assessment & clearancefrom NDNA to play competitive netball.
Division or Age / Team NameCompetition for 2018 will be graded into Divisions – please provide information for your team/s.
Team Name MUST be provided and cannot be changed after Round Robin games.
Please read our Information Letter regarding fees and proposed Fixture Calendar for our competition this year.

FEESAll required fees must be paid by due date as set out in the Information Letter.
The Player Registration Card must be fully completed and signed by each player.
Players registered during the season must ensure all fees are paid.
Teams who are unfinancial will be considered unregistered and penalised accordingly.
Players must be in FULL Uniform at all times. Teams are permitted four (4) calendar weeks from date of registration to be in full uniform.

Players must play a minimum of three (3) competition games to qualify for finals games.
